Euphol, a natural cannabinoid agonist, as a novel potential compound for the
management of inflammatory and neuropathic persistent pain

Cannabinoids have been considered as potent therapeutic agents in pain management.
Central and systemic administration of natural, synthetic and endogenous
cannabinoids produce antinociceptive and antihyperalgesic effects in both acute and
persistent animal pain models. Persistent pains, related to inflammatory and
neuropathic states are prevalent and debilitating diseases, which remain without a
safe and adequate treatment. Here, we reported the discovery of the natural
tetracyclic triterpene euphol, which exhibited pronounced and long-lasting analgesia
when assessed in several rodent behavior models of inflammatory and neuropathic
persistent pain, via activation of cannabinoids receptors, predominantly, CB2
receptors. The results of the present study demonstrate that euphol displays a good
oral absorption, seems to be safe analgesic/anti-inflammatory compound. Its
pharmacological actions are likely associated with the ability to inhibit the release
and/or expression of pro-inflammatory mediators, such as TNF-α, IL-1β, KC, IL-6 and
MPO activity. Oral administration of euphol completely inhibited the mechanical
hypersensitivity induced by CFA injection and this effect was markedly blocked by
either CB1 or CB2-selective antagonists. Of relevance, the intrathecal pre-treatment
with CB1 and, mainly, with CB2 oligonucleotide antisense (AS-ODN, i.t.) consistently
blocked the analgesic effect of euphol. Euphol treatment fails to show CNS activity in
a tetrad of behavioral tests. Our data indicate that mechanisms underlying euphol
analgesia appear to be related to its ability to activate PPARγ transcriptional activity
and by preventing both nuclear factor-κB (NF-κB) and nuclear phospho-cyclic AMP
response element binding protein (CREB) activation, associated with the inhibition of
cyclooxygenase-2 (COX-2) and protein kinase C epsilon (PKCε) expression, either at the
spinal cord or the dorsal root ganglia (DRG) level. Therefore, euphol represent a novel
orally active and safe phytocannabinoid that exhibits great therapeutic potential for
the management of inflammatory and neuropathic persistent pain states.
