1. Reference: a.) Blotter Entry 2015-001

2. On February 24, 2015 at exactly 4:30PM, and alleged shooting incident

transpired at Lopez Ave., Maquiling Subd., Brgy. Batong Malake, Los Baños
Laguna.
3. Maria Josefa Dajes Domingo, 40 years old, College level, House wife,
Filipino, Female, Married, and a resident of 45 Lopez Ave., Maquling Subd.,
Brgy. Batong Malake, Los Ba Ños Laguna reported the incident through using
a telephone call. The victim was identified as Fernando Sagpatan Domingo,
45 years old, college level, Driver, Married, Male and a resident at 45 Lopez
Avenue, Maquiling Subd. Batong Malake, Los Baños Laguna.
4. The victim was washing his car in front of his house when the unidentified
suspect but has a height of 5’6ft shot the victim and then fled at the scene on
a motorcycle without plate number. The victim was immediately brought to
Los Baños Doctors Hospital & Medical Center for treatment of his injury.

2. On February 24, 2015 at exactly 4:30PM, and alleged shooting incident

transpired at Lopez Ave., Maquiling Subd., Brgy. Batong Malake, Los Baños
Laguna

3. Maria Josefa Dajes Domingo, 40 years old, College level, House wife,
Filipino, Female, Married, and a resident of 45 Lopez Ave., Maquling Subd.,
Brgy. Batong Malake, Los Ba Ños Laguna reported the incident through using
a telephone call. The victim was identified as Fernando Sagpatan Domingo,
45 years old, college level, Driver, Married, Male and a resident at 45 Lopez
Avenue, Maquiling Subd. Batong Malake, Los Baños Laguna.

4. The victim was washing his car in front of his house when Francisco Mejia
Fernandez a.k.a. “Boy Negro”, High School graduate, Driver, Co-worker of the
victim, 35 years old, and a resident at 60 Chipeco St. Parian, Calamba City,
Laguna shot him on the chest. A .45 caliber with an unknown serial number
(SN) was found in the possession of the suspect and was confiscated. The
victim sustained 5 gunshot wounds on the chest, he was immediately brought
to Los Baños Doctors Hospital & Medical Center but was pronounced dead on
arrival.

5. Investigators collected 5 empty slugs of .45 Caliber on the crime scene.

6. Suspect is now arrested and will be subject for inquest proceedings and
booking for data gathering of the suspect.

2. February 24, 2015 at exactly 4:30PM, an alleged shooting incident

5. Police officers were in pursuit of the suspect upon receiving info that he
was driving a motorcycle that has no plate number as a getaway.

6. Local police suggested to conduct manhunt operation for the apprehension

of the suspect. The suspect will hereby by held in an inquest proceeding.

7. Upon arrest, it was found out that the motive of the crime was because of
jealousy. The suspect will be filed a case of murder and the trial will be held at
a Municipal Court at Manila S Rd, Los Baños, Laguna.
