SUBJECT : Spot Report re: Alleged Stabbing Incident

DATE : March 26, 2015

1. Reference: a. Police Blotter Entry no. 2015-001

2. At about 6:00AM of March 26, 2015, a citizen reported personally at the

PCP, Barangay. San Antonio, Los Baños Laguna. The citizen is identified
as Ramil Hernandez Ballesteros, 20 years old, college student, Male,
Filipino, Single, and a resident at 50 National Hi-way, Brgy. San Antonio,
Los Baños, Laguna.

3. The incident took place at around 5:30AM of March 26, 2015, an

alleged stabbing incident transpired at National Hi-way, Brgy.
Antonio, Los Baños, Laguna. According to the witness, the
unidentified suspect repeatedly stabbed the chest of the victim
resulting to his death. The victim was identified as Joey Hernandez
Ballesteros, Call center agent, 25 years old, College level, Male,
Filipino, Married, works at Makati and a resident at 51 National Hi-
way, Brgy. San Antonio, Los Baños, Laguna. The witness stated
that the unidentified suspect was wearing a black polo shirt with his
fan knife who left on foot immediately towards an unknown
direction. The victim was brought to the nearest hospital at Health
 Serv - Los Baños for medical treatment, the attending physician
stated that the victim was Dead on Arrival

4. Manhunt operation shall be conducted for the pursuit of the suspect

6. At about 6:00AM of March 26, 2015, a citizen reported personally at the

PCP, Barangay. San Antonio, Los Baños Laguna. The citizen is identified

Los Baños, Laguna.

physician stated that the victim was Dead on Arrival

3. Upon further investigation during the manhunt operation, the

suspect was arrested by one of the operatives of the Los Baños
Municipal Police Station. The suspect was identified as Mario
Agdeppa y Sta. Clara. A Call center agent, Male, 22 years old,
Filipino, Single, works in Makati and a resident at 57 Rizal St., Brgy.
Pulo, Cabuyao City. The height of the suspect is 5’7”ft and weigh
70kgs. The suspect is now under the custody of the Los Baños
Municipal Police Station.

4. Investigators stated that the victim sustained five stabbed wounds

at his chest that caused his death.

5. An autopsy and blood examination of the victim and the fan knife
shall be requested at the PNP Crime laboratory, PRO4A, Camp
Vincente, Lim, Calamba City. 

6. Suspect is now arrested and will be subject for inquest proceedings and
booking for data gathering of the suspect.

2. At about 6:00 o’clock in the morning of March 26, 2015, a

concerned citizen reported personally at the PCP, Brgy. San
Antonio, Los Baños Laguna, identified as Ramil Hernandez
Ballesteros, 20 years old, college student, Filipino, Male, Single,
and a resident in 50 National Hi-way, Brgy, San Antonio, Los
Baños, Laguna, reported and requested for an investigation of the
above cited incident.

3. Upon further investigation during the manhunt operation, the

suspect was arrested by one of the operatives of the Los Baños
Municipal Police Station. The suspect was identified as Mario
Agdeppa y Sta. Clara. A Call center agent, Male, 22 years old,
Filipino, Single, works in Makati and a resident at 57 Rizal St., Brgy.
Pulo, Cabuyao City. The height of the suspect is 5’7”ft and weigh

Municipal Police Station.

4. Laboratory results revealed that the blood stain that was on the fan
knife of the suspect have cross matched the blood of the victim. The
laboratory results shall be used as an evidence against the suspect

5. A murder case was already filed today at the Regional Trial Court,
Calamba City. The suspect is now under the custody of Los Baños
Municipal Police Station for booking of the data of the suspect.

6. It turns out that the motive of the suspect was jealousy, because his
fiancée was married to the victim almost a month ago that which
made him commit the crime of murder.

On the stated date and time, Ramil Hernandez Ballesteros, 20 years old, College
student, Male, Filipino, Single and a resident at 50 National Hi-way, Brgy, San
Antonio, Los Baños, Laguna reported personally that an alleged stabbing incident
transpired at National Hi-way, San Antonio, Los Baños at around 5;30AM. The victim
was identified as Joey Hernandez Ballesteros, a Call center, College level, Male,
Married, Filipino, 25 years old and a resident at 51 National Hi-way, Brgy. San
Antonio, Los Baños, Laguna. The witness stated that the victim was repeatedly
stabbed at his chest by an unidentified suspect which resulted to his death, the
witness also said that the unidentified suspect was wearing a black polo shirt and was
with his fan knife (balisong). The suspect left immediately on foot towards an unknown
direction. The victim was brought immediately to the nearest hospital at Health Serv -
Los Baños for medical treatment but, the attending physician stated that the victim as
dead on arrival.

DATE : FEBRUARY 24, 2015

TIME : 4:30 P.M

SUBJECT : Shooting Incident

On the stated date and time, Maria Josefa Dajes Domingo, college level, house wife,
Filipino, female, Married and a resident of 45 Lopez. Avenue, Maquiling Subd,
Barangay Batong Malake, Los Baños, Laguna, she reported by telephone that an
alleged shooting incident transpired on February 24, 2015 at around 4:30PM. The
victim was identified as Fernando Sagpatan Domingo, College Level, Driver, Filipino,
Married, Male, 45 years old and a resident at 45 Lopez. Avenue, Maquiling Subd,
Barangay Batong Malake, Los Baños, Laguna. The reporter stated that while the
victim was washing his car infront of their house, when suddenly the unidentified
suspect shot the victim. The victim was immediately transported to Los Baños
Doctors Hospita and Medical Center. The suspect escaped by riding a motorcycle
that has no plate number
