This preliminary page is allotted for the work immersion student as the author of

this portfolio on work immersion program. This is to introduce himself/herself by stating

his/her personal information or highlights of his/her achievements in life. The author

must write in third person following the APA style of writing narrative report and

technical paper.

He/she must include personal information like where and when he/she was born,

name of the parents and siblings, educational attainment from kindergarten or elementary

level to senior high school level, achievements like awards and citations received in

different contests, participation and leadership in different clubs and organizations. The

author should observe the proper sequence of events in chronological order. This page

will not limit the author to one page. The author is restricted from mentioning words of

appreciation and gratitude to any person because this page is not intended for

acknowledgment but to give the readers idea about the author’s personal life.

This preliminary page is intended for the author to give credits and words of

gratitude and appreciation to all persons who contributed to the success of his/her work

immersion program. It must start to an introductory sentence like “I convey my sincere

thanks to all the people whose efforts and patience helped in the completion of my work

immersion program and this portfolio”. This page must be written in first person.

Then, the author can now enumerate all the persons he/she wanted to give

appreciation and gratitude such as the Almighty God, family members, faculty members

and staff from Angelo L. Loyola Senior High School involved in the work immersion

program like the principal, partnership focal person, work immersion teacher, class

adviser and other teachers who he/she thinks made significance in the conduct of the

work immersion program, employees from the partner institution starting from the

company owner or the highest officials, department heads and work immersion partner

institution supervisor. The author can also include other personalities he/she would like to

thank like close friends, classmates or newly acquainted friends he/she met in the

workplace. There is no limit for the author to whom he/she wanted to give credits in the

completion of this work immersion program and portfolio.

The purpose of this page is to inform readers and future work immersion students

about the conduct of the senior high school work immersion program as a requirement for

finishing secondary education. The author must describe the content of this portfolio

from the first to the last activity. It is required to include the importance of documenting

the work immersion program by accomplishing this portfolio. The objectives why a

Grade 12 student should conduct the work immersion program should be mentioned. For

the last sentence, do not forget to end this page by writing the initials of the author’s

name as identity-concealing label at the bottom part of this page.
