This prisoner was issued a parole. Per statute, the Parole Board must provide your office with a 28-day notice of the
prisoner’s release on parole. According to the Centers for Disease Control and Prevention (CDC), incarcerated persons
are at a heightened risk for rapid transmission of coronavirus (COVID-19). Prisoners may also have underlying
medical conditions that place them at higher risk for coronavirus (COVID-19), therefore, we are asking that you
consider waiving the 28-day notice of parole requirement so that we may expedite his/her release to avoid exposure to
the coronavirus (COVID-19). These cases do not have registered victims with the Michigan Department of
Corrections.

Please provide your signature on the line below indicating if you approve or deny waiving the 28-day notice of parole
requirement. Please email the completed form to MDOC-FOA-Prosecutor@michigan.gov.

Thank you for your time and consideration.

Sincerely,

Michael C. Eagen, Chairperson

Michigan Parole Board
