HealthLinc Background
HealthLinc provides primary and preventative care services along with specialty services such as dental and optometry
at 8 clinics across Northwest Indiana. The clinics are federally qualified health centers (FQHCs), which means HealthLinc
receives grants under Section 330 of the Public Health Service Act. Because of this, HealthLinc offers a sliding fee scale
for uninsured or underinsured cash paying patients. The patient population at HealthLinc is mainly insured by Medicaid
and Medicare, but many patients are also uninsured or underinsured.
HealthLinc is also able to partner with Walgreens and CVS Pharmacy to offer 340B drug pricing. This allows the
pharmacies to purchase certain drugs at a significantly reduced cost, making them affordable for our patients.

Rotation Hours
● The student is expected to be present in clinic from 7:50am to 4-5pm Monday through Friday throughout the 4-
week rotation block, unless otherwise specified. Any absences from the rotation must be approved by the preceptor
in advance.
● Any additional responsibilities will be outlined on the student calendar and discussed at the start of rotation.
● NOTE: If patient care responsibilities are not complete for the day—additional hours may be required to fulfill these
responsibilities.

Required Activities/Assignments
COUMADIN CLINIC
● Review CHEST guidelines during Week 1 of rotation
● Review anticoag folder on U drive during Week 1 of rotation
ACADEMIA
● Depending on the time of year, students will take part in teaching a portion of lectures at Purdue. Lecture topics
will vary—preceptor will review expectations and dates of lectures during orientation.
ASSIGNMENTS
A. CV Review –Student will email preceptor a copy of their CV before the start of rotation. During orientation
preceptor will evaluate CV and discuss feedback with the student.
B. Project—Student will develop a long-term project of their choice to provide educational information, enhance
patient care, improve medication safety, or strengthen other pharmacy services. The project is due by the final
week of the rotation. Examples: compare/contrast chart of insulins, diabetes education materials, insulin sensitivity
factor flowsheet for patients, etc.
C. Journal Club—Student is responsible for preparing a journal club for presentation—the student is responsible for
choosing an article to present. The article chosen should be sent to preceptors to read at least 1 week prior to
presentation.
D. Topic Discussions—Student will be responsible for preparing 2 topic discussions to present to preceptors
a. Examples
i. Diabetes – oral drugs, injectable drugs, insulin, clinical pearls, U-500, injection technique
and demonstration, insulin pumps, symogi effect, dawn phenomenon, etc.
ii. Anticoag – DOAC, warfarin, clinical pearls, indications of anticoag drugs, etc.
iii. Other Ambulatory Care topics
E. Note for your successor—Many of the patient care tasks during this rotation will not be complete before you leave.
Student will be responsible for writing a note to the next student to ease their transition into the rotation.

● “HealthLinc, this is (insert your name). Can you hold please?” is an appropriate phrase for answering the phone if
necessary. Though, you probably will not have to answer the phone.
● To make an outgoing call—first dial “7” followed by the phone number. Depending on the phone number dialing “1-
area code” before the phone number may be necessary.

● Local fax numbers may be dialed without an area code
● Just like a phone call, depending on the recipient, “1-219” before the fax number may be necessary.
● For long distance numbers dial “1” followed by the area code and number
