4/9/2006 945 Nexen During the transfer of synthetic oil based mud to the rig GSF Arctic I, the motor vessel "Cape Hope" lost power and floated under the N N N
Petroleum rig. There were no injuries, but there was considerable damage to the rig and the vessel. The Coast Guard's Morgan City Marine
U.S.A. Inc. Safety Office is the primary investigation Agency in this incident.
4/19/2006 1300 Energy Partners, A workboat had delivered supplies to Pride Georgia drilling rig and moved approximately 150 yards from rig. The boat captain then N N N
Ltd. shut down the engines and went to the rest room. The boat then drifted back and hit the stern leg of the rig, causing a dent in a
cross member of the leg. The #1 well had just been completed and the rig was securing equipment to move off location when the
incident occured. There was no damage to the well, no injuries or fatalities, and no pollution.
5/2/2006 1000 PetroQuest A shrimpboat trawling near the platform, at full throttle, struck the platform. The vessel, M/V St. Vincent, damaged three boarding N N N
Energy, Inc. flowlines coming in from nearby wells; the flowlines were not pressurized at the time of the incident. The damage was minor with no
fatalities, injuries or pollution.
6/3/2006 2400 Chevron U.S.A. A pleasure boat collided with platform during night hours. The vessel collided with the western side of the platform at the +10 level. N N N
Inc. It was reported that there were no operable nav-aid lights or fog horn on the platform. The EI 297 A platform is an unmanned
structure and there were no injuries or pollution reported as a result of this incident. The vessel was a 43 feet by 13 feet beam and 6
feet depth fiberglass sail boat; it was under power with the main sail up when the collision occurred. The vessel sustained critical
damage and required USCG assistance to stay afloat.
7/11/2006 2020 El Paso On July 11, 2006 at 8:20 PM, the M/V International Thunder struck El Paso’s WC 504 C platform while in route from HI 389 to the W&T Base in Cameron, La. No N N N
Production injuries have been reported, and the vessel continued on to the W&T base after the collision. No leaks or spills were reported. The M/V International Thunder is a
145’ supply boat and was contracted to W&T Offshore at the time of the incident. Initial reports indicate that damage occurred to the caisson platform’s boat
landing, boat bit and pipeline riser clamp. The riser clamp was knocked loose from the caisson. The platform had been shut-in before the incident, but the
departing pipeline had pressure and had recently been put back in service. According to the boat’s Captain, the vessel struck the platform on its port bow as he
steered to starboard in an attempt to avoid collision.

7/14/2006 1400 Mariner Energy, At 1400 hrs, a shrimp boat, the "Lucky Tommy" struck Mariner's WC 163, #3 platform and severed a natural gas flow line connected N N N
Inc. to the platform. The platform's safety system shut-in the well. The severed flow line produced a condensate sheen with dimensions
of 100' x 20'. No injuries were reported.
7/23/2006 1200 ExxonMobil Oil While conducting water transfer operations, the motor vessel "Miss Adrian" drifted into the structure. No damage to the structure N N N
Corporation was reported with minimum damage to the motor vessel.

7/30/2006 1445 Apache A pipelay barge, Crossmar 14, was being towed by the motor vessel "Miss Jessica" using the number 1 anchor cable as the towline N N N
Corporation and was tending off the port bow (left forward section of the barge). As the barge approached the predetermined anchor locations,
northwest of the platform, the number 3 anchor was released and was tending off the starboard stern (right aft side of barge). The
“Miss Jessica” continued forward at approximately 4 knots according to the tug captain. Slack was payed out on the number 3 anchor
to a distance deemed necessary to stop the vessel. The brake was set to stop the vessel's forward momentum, at which time the
number 1 anchor was set by the tug. As the cables became taut, the barge pivoted (due to being anchored on opposite corners on
the bow and stern) and struck the B-6 well conductor in two locations. The collision caused the conductor to bend inward and split
the weld seam at the point of contact with the barge's helideck. The anchor bolster of the barge made contact with the structures
horizontal support beam approximately 3 feet above the waterline leaving an indentation.

7/31/2006 1425 Newfield While performing routine production checks in the field, the M/V "Weezer" was backing up to the structure when the vessel's throttle N N N

Inc. turning his bow into the current but misjudged his position, damaging the mask, both fire monitors, buckling his deck, and crippled
his radar. He also damaged the east mooring lay down area on the spar.
8/11/2006 230 Mariner Energy, The motor vessel "Gary John" backed into the lift boat "Aries Ram XI" on the port side of the aft leg. At this time, damage has not N N N
Inc. been fully accessed and a determination has not been made as to whether or not repairs will be necessary.

8/18/2006 2300 Apache The motor vessel "Seahorse V" collided with the GI 40-M platform. The "Seahorse V" was on stand-by pollution recovery service for N N N
Corporation BP's sunken GI 47-C platform. The GI 40-M is an unmanned platform and was shut-in at the time of the collision. BP notified Apache
of the incident. Initial damage includes two bent well heads, with the riser of one well being pulled out of the bell guide. No
information is available on the damage to the Seahorse V at this time.

9/13/2006 1800 Chevron U.S.A. During a daily tour of the field, operators discovered damage done to the facility, apparently by fishing vessels tying off to the N N N
Inc. handrails. The handrails on one side of the lower deck were pulled down far enough to create a potential hazard to personnel who
would board the facility. The facility was placed on the "not safe for personnel to board" list until repairs were made.
10/8/2006 1905 Chevron U.S.A. A crew boat was backing up toward the lift barge. The current caught the crew boat forcing it into the stern of the lift barge. Upon N N N
Inc. impact the fuel tank in the hull of the lift barge was ruptured. No diesel spilled and damage to the lift barge and crew boat was under
$25,000.00.
10/9/2006 1830 Chevron U.S.A. An unknown marine vessel collided with the single well satellite structure. Well No. 138 sustained substantial damage. A production N N N
Inc. process system shut down alarm sounded on the SM 236 A host facility followed by the activation of a PSL shut in sensor on a
compressor final discharge and departing gas lift line which supplies gas lift gas to the satellite wells in the SM 236 field. Platform
personnel on the host facility responded to the alarm, shut in the well, and dispatched the field boat investigate. The field boat
discovered that the wellhead tree for well No. 138 had been sheared off above the tubing hanger, and dry gas was escaping from the
incoming ruptured gas lift line. Operator personnel on the host facility reported that several shrimp boats were in the SM 236 area.
The jack-up boat "Christie" was mobilized to install a buoy at Well No. 138 on October 11, 2006, and a work boat was assigned to
remain on standby near the damaged wellhead until the weather improved so that repair operation could be initiated. Dry gas only
was released from the ruptured gas lift gas lift pipeline. There were no injuries or pollution reported as a result of this incident.

10/16/2006 515 McMoRan Oil & An outrigger boom on the shrimp boat "John Paul" hit the drilling rig. The boom contacted the aft escape ladder on the rig bending it N N N
Gas LLC badly. The "John Paul" then moved to the amidship starboard side and hit a starboard column brace and slightly bent the sounding
tube for a ballast tank.
10/30/2006 1350 Maritech While supplying water to the platform the motor vessel "John" rubbed against something below the water causing a gash in the hull N N N
Resources, Inc. just above the water line. The vessel went Fourchon and was accompanied by the motor vessel "Leah." The damage was not severe
enough to pose any threat to the vessel sinking. Maritech will investigate to see what may have caused the gash in the vessel's hull.

11/11/2006 1500 Offshore Energy EI 88 Caisson #1 and Caisson #2 sustained substantial damage when the motor vessel "Candy Dancer" collided with the structure. N N N
I LLC Operators arrived at the facility at approximately 1430 hours on November 12, 2006 and found the foghorn and safety panel were
ripped away from the structure and the boat landing was severely damaged. The well had shut-in and a small amount of hydraulic oil
that leaked into the Gulf from the safety panel.
11/15/2006 1800 Taylor Energy The Liftboat Hercules "Tiger Shark" was on location, jacked-up, and pre-loaded. The legs of the liftboat penetrated the sea floor N N N
Company deeper than expected. The liftboat was then sitting 18 feet out of the water. A weather front was moving towards the area. Taylor
management decided to shut in the platform and bleed it down when seas got to 13 to 14 feet. All personnel were removed from the
liftboat and returned to SS 229 "A" for the night. During the night, the lights from the boat were no longer visible from SS 229 "A".
The next morning, the boat was no longer on location. The crew boarded SS 242 "A" and discovered damage to the second deck on
the Southeast corner of the platform. The liftboat was located adrift and upright in ST 292, approximately 40 miles off location.
11/12/2006 1500 W & T Offshore While conducting an inspection of the EI 88-A single caisson platform, operators discovered that the platform was damaged by a marine vessel of unknown origin. N N N
The platform sustained substantial damage and was shut in.

11/22/2006 1815 Comstock A Motor Vessel (MV) collided with a wellhead satellite. MV personnel indicated that the light was not visible on the wellhead and the N N N
Offshore, LLC structure was not visible on radar. However, after the collision, the light came back on. The vessel received a 2 ft gas high on the
port bow. A pipe on the wellhead broke off resulting in a gas release into the air. The operator reported that Platform #16 does not
have any producing wells and is a crossover jacket. The collision broke a riser on Pipeline Segment 9040, which is a gas lift pipeline.
At the time of the collision all producing wells were shut-in. The operator stated that the Nav-Aids were working at the time of the
incident.
12/13/2006 1715 Nippon Oil The supply boat "Kline Banos," backed into the bow leg of the Pride Kansas at 1715 hours on 12/13/06. There were no injuries or N N N

12/14/2006 1620 BP Exploration The Transocean "Marianas" MODU suffered damage to its starboard aft A column during mooring operations in the BP Nakika field. N N N
& Production The Marianas had just transferred the # 3 anchor wire to the Seacor Anchor Handling Vessel (AHV) "Gerard Jordan" and was in the
Inc. process of hanging off the wire's PCC when the AHV's stern struck the rig. All work was immediately stopped and the incident was
reported to the Marianas offshore installation manager and BP WSL's. The decision was made to disconnect the anchor wire from the
AHV so it could move to a safe distance from rig. The wire was disconnected safely and the vessel was relocated a safe distance
from the rig. The rig was ballasted up to 19 meters to assess damaged area. The Marianas Marine team was instructed to open
entry hatch to the starboard aft column and start ventilation of area to allow further inspection from inside. The damage to the rig
appears to be a mash or dent approximately 10' long x 6' high x 12" deep, and there does not appear to be any penetration of the
steel column. The rig was being moored in preparation to re-enter and resume drilling a well.
