Brief details of incident:


Patient was prescribed Amoxicillin 500mg/7h PO, despite ampicillin alergi (this was stated
on medicinal content). This was noted by the pharmcistand the patient did not take any
Amoxicillin. The team PRHO was contacted and an alternative antibiotic was prescribed.

1. Where do antibiotics come from?

Answer: Antibiotics come from natural sources these are used to battle against bacteria out in
the environmental,scientists take these molecules and use them to help fight infections in
people.

2. What are some types of bacteria?

Answer: There are two types of bacteria in the body, there are good ones and bad ones.

3. What is microbiome?

Answer: Everyone's bacteria have something called a microbiome. Our micribome is a collection

of bacteria that live in our body. Strong microbiomas are microbiomas that have many types of
good bacteria that protect us from getting sick so it is important for us to take care of these

good bacteria but sometimes it remains bad.

4. When do we need antibiotics?

Answer: Sometimes bad bacteria can get in cause problems , this is why you have been

prescribed antibiotics

5. What is meant by targeted and broad antibiotics?

Answer:

 Targeted :Sometimes, when you are sick, your health care provider knows exactly what

the bacterium is. It causes your illness. At times like this, targeted antibiotics are

prescribed.

 Boad: This type of antibiotic mainly kills harmful bacteria however, sometimes, your

health care provider may not know exactly what is causing your illness, if your health

care provider suspects it could be a bacterial infection, antibiotics are widely prescribed.

6. How to find the best antibiotics?

Answer: to find the best antibotic for you ,it's important that your healthcare provider knows
what you're allergic to and other medication you may be taking.

7. What are allergic reactions?

Answer: it's important to know what an allergic reaction is and tell your doctor about the

allergic reaction.

These include:

 Lips, throat and floating eyes.


 Low blood pressure.

 Skin rash.

8. Is it important to know difficile infection?

Answer: difficile infection is an illness that occurs in hospital institutions and patients who are
on or have had antibiotic treatment.it is very ,very serious. If you experience symptoms of
watery dirrhea and / or abdominal "belly" pain ,and possibly fever ,you need to contact your
helathcare provider immediately.

9. What important questions should we ask to the doctor when the doctor prescribes
antibiotics for us?

Answer:

1. Why am I being prescribed an antibiotic?


2. How should I take my antibiotics and for how long?
3. How will I know if my antibiotics are working and what side effects should I watch for?
4. Do I need any tests and when do I book my next visit?
