EMBASSY CHECKLIST - MVV VISA APPLICATION

• Arrive 5 – 10 minutes prior to your appointment;

• Besides the original documents, bring photocopies of all originals, including a copy of the
holder page of your passport and residence permit/visa;
• Make sure your passport application form is flied out and completed before you arrive. The
form is to be signed during your appointment.

Mandatory documents – Knowledge Migrant

Original documents and a copy of each document must be submitted. If no Checkbox
copies are submitted, original documents will not be returned.
1. Passport:

• At least valid for six months;
• Two blank visa pages;
• Issued no more than ten years ago.
2. Valid proof of residency in the country of application (passport/visa/permanent)

3. One recent passport picture (preferably no more than 6 months old) according

the Dutch passport requirements. Please find the requirements attached.
4. Completed MVV visa application form. Please sign the form at the

embassy/consular office.
5. A copy of the IND letter regarding the issuance of the MVV visa.

Mandatory documents – Residence with spouse/registered partner/unregistered partner

Original documents and a copy of each document must be submitted. If no Checkbox
copies are submitted, original documents will not be returned.
1. Passport:

• At least valid for six months;
• Two blank visa pages;
• Issued no more than ten years ago.
2. Valid proof of residency in the country of application (passport/visa/permanent)

3. One recent passport picture (preferably no more than 6 months old) according

the Dutch passport requirements. Please find the requirements attached.
4. Completed MVV visa application form. Please sign the form at the

embassy/consular office.
5. A photocopy of the page(s) in your partner’s passport or identity document

showing his/her personal details and, if already obtained, a photocopy of your
partner’s residence permit.
6. Legalized/apostilled and translated marriage certificate. OR

Legalized/apostilled and translated non-marriage certificate. This declaration

must have been issue within the past six months.
7. A copy of the IND letter regarding the issuance of the MVV visa.
