sBased on observations and staff interviews, the facility fale to provide a safe, sanitary and comfortable environment to help prevent the potential
evelopment and transmission of disease and infection. Specifically, tne facility failed to clean an area contaminated with a bodily fluid ina timely manner.
‘The findings include:
A. Observations
‘continuous ebservation began an 1/27/16 at 8:55 a.m., Resident #59 was observed as he propelled his wheelchair from the dining room across the
Luncarpeted floor in the common area directly outside of the north nurses station. The residents feet and legs were wrapped with elastic bandages and
covered with paper fabric booties. As the resident moved across the floor, a wet substance trailed his foot as it touched the ground. A certified nurse alde
Was informed of the situation and saié she would check on the resident who had entered his room and see to the floor
‘At 9:00 a.m. a balloon toss activity occurred inthe north wing common area. Several times the balloon left the circle and landed on the floor next to the area
were the resident had passed through. The balloon was placed back inte play and was touched by 8-10 residents and the activity staf who facilitated the
activity,
Numerous other residents passed through the area as they returned to their rooms fram the ining room,
‘A: 10:06 a, m., a nurse at the north nurses’ station was asked about the resident’ status. The nurse said the lui was likely edematous exudate that had
‘azed from the resident’ right leg and was not urine, The nurse sal the resident was under Isolation precautions for a Methelliln-resistant Staphylococcus
aureus (MRSA) infection on his eft leg but thought he was nearly through with his antibiotic treatment. The nurse sald she would have someone clean the
floor. interviews
(On 1/27/16 at 11:25 a.m. the assstant director of nursing (ADON) and director of nursing (DON) were advised of the previous observations. Both the ADON and
DON said any biological fulds spilled on the floor should be cleaned up as soon as staff were made aware of the spill. The DON said leaving the spill on the
floor Increased the risk of cross contamination with other residents. The ADON and DON sald they would address the issue with the nurse and have
housekeeping clean the spil immediately.
‘The staff development coordinator (SDC), who was also responsible for the infection control program, was interviewed on 1/28/16 at approximately 3:30 p.m.
“The SOC said any biological secretion or fivid should be cleaned and the area sanitized whenever staff area aware ofthe spill, The SDC said the rsk of cross
contamination and spread of infection was increased when staff were notified but dis not immediately clean the wet floor. She said she would Inservice the
staff regarding this issue right away.
C. Follow-Up
“The spill was cleaned and sanitized by the environmental director (ED) on 1/27/16 at 1:30 p.m.
