EVALUATION GUIDELINES FOR MASTER TEACHER POSITION


I. Basic Requirements Documents/Supporting Papers
1. Permanent Teacher 1.1 One certified zerox copy of CSC/PBET or
PRS/LET rating/R.A. 4670
1.2 One certified xerox copy of Appointment
Paper/Service Records/Certification signed by the
Superintendent or Administration Officer.
2. Bachelor’s degree for 2.1. One certified xerox copy of transcipt of Records
Teachers or its Equivalent as with statement of Graduation and/or Special Ordeer for
