I. Basic Requirements
1. Permanent Teacher
2. Bachelor’s degree for
Teachers or its Equivalent as
provided for in Magna Carta
MT-I 18 MA units
MT-II CAR
MT-III Ma or 30 years
experience + 20 MA units
Documents/Supporting Papers
1.1 One certified zerox copy of CSC/PBET or
PRS/LET rating/R.A. 4670
1.2 One certified xerox copy of Appointment
Paper/Service Records/Certification signed by the
Superintendent or Administration Officer.
2.1. One certified xerox copy of transcipt of Records
with statement of Graduation and/or Special Ordeer for
graduation if graduate of a private school
2.2. Certification from the dean of Graduate
School/Transcript of Records

3. Very Satisfactory rating
for the last three (3) years (at
least 8.3 Performance Rating)
4. Experience

Head

6.1 One copy of Teacher Program duly signed by the
principal/District Head

Requirements for Demonstration Teaching

1. Certification (1 copy from concerned school authorities indicating the rating among
other things.)
2. Copy of the lesson plan approved by the School Head
