GENERIC NAME: Methylergometrine maleate
BRAND NAME: Mergot
CLASSIFICATION: Ergot alkaloid
DOSAGE: Secondary postpartum hemorrhage 125 mcg tid for 3 days.
Prevention & treatment of postpartum hemorrhage 200 mcg IM repeated if
necessary at intervals of 2-4 hr
MECHANISM OF ACTION: Increase tone, rate, and amplitude of contraction of uterine
smooth muscle.
USES: Management of uterine atony, haemorrhage and subinvolution of uterus following
delivery of placenta.
INDICATION: Postpartum hemorrhage. Routine management after delivery of the
placenta, postpartum atony, hemorrhage, uterine subinvolution.
CONTRAINIDCATION: Induction of labor or 1st stage of labor. Patients w/ eclampsia
SIDE EFFECTS: Frequent: Nausea, uterine cramping, vomiting. Occasional: Abdominal pain,
diarrhea, dizziness, diaphoresis, tinnitus, bradycardia, chest pain. Rare: Allergic reaction (rash, pruritus),
dyspnea; severe or sudden hypertension.
ADVERSE EFFECTS: Severe hypertensive episodes may result in CVA, serious arrhythmias, seizures.
Hypertensive effects are more frequent with patient susceptibility, rapid IV administration, concurrent use
of regional anaesthesia, vasoconstrictors. Peripheral ischemia may lead to gangrene.
NURSING CONSIDERATION
BASELINE ASSESSMENT
Determine baseline serum calcium level, B/P, pulse. Assess for any evidence of bleeding
before administration.
INTERVENTION/ EVALUATION
Monitor uterine tone, bleeding, B/P, pulse q15min until stable (about 1-2 hrs). Assess
extremities for color, warmth, movement, pain. Provide Support with ambulation if dizziness
occurs.
PATIENT/ FAMILY TEACHING
Avoid smoking: causes increased vasoconstriction. Report increased cramping, bleeding,
foul-smelling lochia. Report pale, cold hands/feet (possibility of diminished circulation.
