The Annual Registration Renewal Fee Report submitted on behalf of the captioned organization
for the fiscal year ending 12/31/2018 is incomplete for the following reason(s):

1. The IRS Form 990, 990-EZ or 990-PF submitted does not contain a complete copy of Schedule B,
Schedule of Contributors, as required. Contributor names and addresses must be submitted along
with contributions. The copy of the IRS Form 990, 990-EZ or 990-PF, including all attachments,
filed with the Registry must be identical to the document filed by the organization with the Internal
Revenue Service. The Registry retains Schedule B as a confidential record for IRS Form 990 and
990-EZ filers.

In order to remain in compliance with the filing requirements set forth in Government Code sections
12586 and 12587, please provide the requested information, together with a copy of this letter, to the
above address, within thirty (30) days of the date of this letter.
