1: On processing of the application form the following shortcomings came to notice

Applicant posted in balod call for employer cert with present residential address proof

Please submit one more proof of address as per table 3 of section d of the passport instruction booklet
available with the new passport application form

If you live in the vicinity of passport office raipur or have been specifically asked to appear in person, you
could choose to submit the required documents to the enquiry office at the passport office on between
10.30 am to 1.0 pm on all working days. if the service is rendered please ignore this letter or please treat
this letter as cancelled.

2: This is to inform you that regarding LBE for north precast area for handling and preservation of Epoxy
coated Rebar (Installed and stored fabricated Rebar) today I visited there and found that there was no
action taken by Jiangxi yet from construction site. See attached photos for your reference.

3: During site surveillance it was observed that SEPCO construction team started CPT (Cone
Penetration Test) Building D-204 Near Grid Line E Drilling work and Near D-201 CPT work at
PKG#6 without RFI which is violating the Approved ITP SACTIP-A-113-01 & SCHEDULE "Q" Para
1.8 which stated that For any inspection or any test required to be witnessed by COMPANY standards,
including any required by applicable laws, rules and regulations of Saudi Arabia, CONTRACTOR shall
provide notice of said inspection or test as specified in the approved Inspection and Test Plans (ITP's)
and Vendors' Inspection and Test Plans to enable the Company Representative to attend. If any part of
the WORK or the FACILITIES is closed or covered before the required inspection or witnessing has been
performed or without agreement by COMPANY, it must, if required by COMPANY, be opened or
uncovered for inspection or witnessing and re-closed or recovered at CONTRACTOR's expense.

4: PID-Routine surveillance was carried-out at SEPCO/SINOHYDRO Laydown No. 2 area, observed


contractor have started fabrication of precast manholes. Contractor is being reminded that if contractor
wishes to precast any elements which are shown on the drawings as in-situ concrete, contractor shall
submit to Company all calculations and layout and detail drawings necessary for the manufacture and
installation of the precast elements and for the completion of each section of the works to satisfy the
original design requirements. All these calculations and drawings shall be approved by the Company
before any work is commenced on the manufacture of the elements. Likewise, Contractor are required,
before commencing work on precast concrete to prepare layout drawings of each section of the work
showing elements to be produced. Detail drawings shall be prepared for each type of unit showing in
detail the grade of concrete, finishes, joint details, reinforcement, joggles, mortices for dowels, clamps,
cramps, lifting hooks, etc. These drawings are to convey faithfully the Company's intention and are to be
submitted to the Company for approval before production of elements begins. Contractor is advised to
comply with the Project Specification RE-614005, Sec. 4.23 requirements to avoid impending violations.

5: PID-Contractor is being reminded that precast concrete elements shall be lifted or supported only at
designated lifting and support points. Provisions for all lifting hooks, etc. are required for handling, lifting
and erecting of precast units in position. All such lifting and fastening devices on precast elements shall
be subject to approval of the Company and shall be shown on the drawing prepared by the contractor
for the Company's approval. Contractor is advised to comply with the Project Specification RE-614005,
Sec. 4.23 requirements to avoid impending violations.

6: PN - SAC-MYP-QA-PRO-0016 (Special Process procedure for Concrete in-situ AND Precast Concrete
Installation Para 4 Responsibility) Which stated that the site civil engineer (Area in-charge) will ensure
the work activities are performing according to this procedure, latest project drawings and specification.
It is also his responsibility to make sure the work is ready for inspection and inform to QC inspector
through relevant Request for Inspection and checklist that inspection is required as specified in
approved ITP. Construction shall not proceed work for next activity without the inspection by QC
inspector and company approval. Company shall be notified 24 hours in advance for the inspection or
test through the RFI.

7: PN - SAC-MYP-QA-PRO-0016 (Special Process procedure for Concrete in-situ AND Precast Concrete
Installation Para 4 Responsibility) Which stated that Site Supervisor directly responsible for the
implementation of safety requirements as well as concrete in-situ procedure to the work force and to
make sure that relevant work is delivered accurately. Prepare construction report to successfully
accomplish the specific task direct supervisor on the work force and implementation of proper
construction standard and procedure. Ensure that smooth work transition at all time is being carried out
during the construction period.

8: PN - SAC-MYP-QA-PRO-0016 (Special Process procedure for Concrete in-situ AND Precast Concrete
Installation Para 7.10 Precast Concrete) Which stated that The Contractor will be required, before
commencing work on precast concrete to prepare layout drawings of each section of the work showing
elements to be produced. Detail drawings shall also be prepared for each type of unit showing in detail
the grade of concrete, finishes to concrete, joint details, reinforcement, joggles, mortices for dowels,
clamps, cramps, lifting hooks and the like. These drawings are to convey faithfully the Company's
intention and are to be submitted to the Company for approval before production of elements or molds
etc. begins. The Contractor shall also supply to the Company, as soon as possible after the start of the
Contract, a schedule of weights for each type of element.

9: PN - SAC-MYP-QA-PRO-0016 ( Special Process procedure for Concrete in-situ AND Precast Concrete
Installation Para 7.10 Rejection of Elements) Which stated that All finished elements, whether erected in
position or not, which do not comply with the Drawings and Specifications shall be removed and
replaced with new items to the Company's satisfaction. Reasons for rejection of units shall include the
following:

(a) The presence of cracks or repairs

(b) The presence of broken edges whether reinforcement is exposed or not.

(c) Concrete cover to any reinforcement being less than that required by this Specification.

(d) A surface finish inferior to that indicated on the Drawings and/or defined in this Specification.

(e) Out of tolerance dimensions.
