All Candidates applying for said examination must complete Step-1 and Step-2 of their online
examination application on or before the dates mentioned above. After completion of Step-1 and 2
all communication regarding eligibility of your exam will be communicated to your e-portal/email.

3 EXAMINATION FEE
Candidate may submit their examination fee using following payment options
- CPSP UBL Booth, Karachi, Lahore, Peshawar Examination Fee: Rs.15,000
- Designated branch of UBL as mentioned on enclosed fee challan
- Online payment using Debit/Credit Card Handling Charges: Rs.50
- Bank draft/Pay order payable in the favour of CPSP
IMPORTANT NOTE: Total: 15,050
Fee submitted other than designated branch of UBL will not be acceptable.
Upload scan copy of your pay order/D.D and submit original to CPSP Regional Centre or Finance Dept.

4 REFUND OF FEE
Fee shall not be refunded nor carried over to the next examination under any circumstances, after the closing
of last date for Withdrawal of applications. 75% of the fee (excluding late fee) may be returned under the following
circumstances.
1: Application for Withdrawal is received within the specified date of Withdrawal.
2: Rejection of application on account of ineligibility.
3: While applying for REFUND candidate should write his / her NAME, CENTRE, SUBJECT and BANK RECEIPT
NUMBER.
5 TRAINING COMPLETION CERTIFICATE & WORKSHOPS
Training certificate should be upto June 30, 2020 must be signed by the Supervisor and Head of Department,
countersigned by Head of the Institute.

All mandatory workshops should be completed before appearing examination

Workshops attended after the due date will not be accepted in any case.

7 SUBJECTS FOR EXAMINATIONS

Candidates who qualify in Theory examination can appear in three out of four consecutive TOACS
examinations (including missed chances) without appearing in theory examination again.
A candidate who does not appear in the immediate following TOACS examination after passing theory
examination will lose one chance. Only two of three consecutive chances will be available to such candidates.

8 ONLINE APPLICATION FORM

Examination forms are available on CPSP website: www.cpsp.edu.pk. Candidates are required to
fill online applications.

9 VENUE OF EXAMINATION
Computer Based examination is conducted at CPSP Karachi, Regional Centre Lahore, Islamabad, Multan, Faisalabad,
Hyderabad, Quetta & Peshawar.

Conventional examination of Operative Dentistry, Orthodontics, Pathology & Psychiatry will be conducted in all Regional
Centres.

The choice of the city should be clearly written in the appropriate box in the application form.

The CPSP reserves the right to delete a particular centre, or shift the candidates from one Centre to another on account
of logistic reasons.

TOACS: The College will hold TOACS examination ONLY in one or more cities of PAKISTAN depending upon the number of
candidates eligible to take this examination and logistic facilities available in that city irrespective of the venue at which a candidate
appeared in theory examination. Exact Venue of TOACS examination will be communicated after the assessment of theory examination
papers.
