FACTS OF THE CASE:

(PHYSICAL INJURIES): One ROMMEL MORALES y SALES, 16 years old,

Single, of Purok-3 Libertad, Paseo Arcenas Banawa, Cebu City 6000
Philippines, appeared this station and reported that he was allegedly
physically injured by DENCE RAY OTIS y MANGUBAT, on or about 10:00
a.m. this date.

Entry Nr. 01-1137/3 Jan 16/4:30PM: Person of ROMMEL M. SALES

appeared this office and reported that the suspect who Physically
injured him was DENCE RAY O. MANGUBAT

09- 1141/4 January 2020/5:00PM: (RE: 091135): Initial

investigation conducted by this station at the place of incident
disclosed that ROMMEL M. SALES and DENCE RAY O. MANGUBAT had an
altercation outside the residence of Mr. Mangubat in the same
barangay, Sitio Libertad Banawa Cebu City, wherein said suspect
physically assaulted private complainant. Incident occurred on or
about 10:00AM this date.
