I, AAAA, Filipino, of legal age, single, with address at Paranaque City, after
having sworn in accordance with law hereby depose and state that:

1. I am the owner of a business establishment named AAAA

GARMENT, located at Paranaque City;

2. Sometime in 1996, I closed the said business due to lack of

manpower and funds for continuous operation;

3. I am executing this affidavit to attest to the truth of the

foregoing and for whatever legal purpose it may serve.
