Re: Request Letter for Cancellation of Business Name

To whom it may concern:

This refers to the Certificate of Business Name Registration issued to me by the Department of
Trade and Industry (DTI).

On 6 January 2016, I was issued a Certificate of Business Name Registration with No. 03841684
for my business Two H3 Tailoring, in Socorro, Oriental Mindoro. The said certificate states that
the registration of business name is valid until 6 January 2021.

However, due to management prerogative and change in line of business, I decided to close the
abovementioned venture. In this regard, I respectfully request from your good office to cancel
the business name registration of Two H3 Tailoring.

Thank you for your usual attention and I look forward to your favorable action regarding this
matter.
