1) Dynamic Form Building:

a. Have UI like Google form

b. Questions creation tools-MCQ, Short description, only number, compulsory fields,
logic questions. Logic Questions: if the answer is X show the next question based on
the previous response.
c. In each form have phone number as compulsory field- If the phone number is
already present in the DB then auto fill that form. Have submit button and display a
message after submission.
d. Customer message to be shown after the form is filled to the user.
e. Unique sharable link for the event can be generated. Tracking system for the same
as to know the source and number of clicks on the link.
2) User Management:
a. We should be able to mark a user journey. Whether a user is a Non PH, PH, if PH
then which level(Basic, Advance, Soul, Psychotherapy) or AY. Again for each user we
should be able to see how many events/classes they have attended/participated
within a particular period of time or overall.
b. IF a Non PH, wants to become a PH, then a class information form has to be filled
either using the Non PH credentials already present in the database which can be
tracked by phone number or through manual new entries. The Form for class has
been attached. The form has to be exportable in PDF.
c. Reference: we need to track who is bringing new attendees or sending prospective
PH to classes. Hence whenever a event or class form is generated, a compulsory field
should also be to know the reference. Reference can be like: Newspaper, Pamphlet,
Name of a previous event, Name of a person in our database, Online Ad, etc. This
reference field can also be manually updated.
d. Also we should be able to update our previous/existing database of users into this
system. Also we should be able to export users database in excel and also integrate
google authenticator like system whenever a system for export for user data takes
place.
e. Based on data filters we decide, we click be able to checkmark users to whom we
want to send emails and SMS. Email list to be created and emails to be sent using
either native gmails or CRM software like mailchimp/Zoho etc. For SMS, SMS
gateway provider has to be integrated and SMS sending/creating and tracking
function has to be integrated.
3) User Analytics:
a. Generate Interactive Timeline for showing individual and group journey.
b. Have in place filter/sorting mechanisms like google analytics for showing/comparing
each data point.
c. Unique data to be displayed on Dashboard: Average Duration a person/student
takes to journey from being a Non-Ph to different PH classes and finally to PH.

b. Dob
c. Phone No. (Master)
d. Email Id.
e. Address: Full Address with Pin code
f. Gender
 g. Profession: Doctor/Lawyer/CA/etc.(Custom/list of profession can be added &
updated)
h. Source/Reference: If Individual- Then name of the person. Newspaper/Name of
Newspaper, etc.
i. PH: Y/N (if Y ask if AY: Y/N). if PH=N then form is complete! Message to be shown
j. Since when PH/AY: Year
