Introduction

A birth certificate is a vital document necessary for a number of things – from getting enrolled in
a school to obtaining different forms of identification. The process of getting PSA birth certificate
was paper-based and high on effort for parents and especially the SHS students of Potungan
National High School.

PSA birth certificate of SHS students in Potungan National High School is essential for them to
graduate high school.
According to the main website, The Philippine Statistics Authority is mandated to be the central
statistical agency, and the agency that will implement all the objective and regulations of the
Republic Act 10625 or the Philippine Statistical Act of 2013 which is described as “An Act of
Reorganizing The Philippine Statistical System, Repealing for the Purpose Executive Order
Numbered One Hundred Twenty-One, Entitled “Reorganizing and Strengthening the Philippine
Statistical System and for other Purposes. The Online PSA birth certificate form will give a big
impact to the people and also prevalent to the SHS students of Potungan National High School
which will give them convenient way to get their birth certificate through fill upping the
application form in using online.

registrar or other custodian of birth records, who is commissioned by the government.
The right of every child to a name and nationality, and the responsibility of national governments
to achieve this are contained in Articles 7 and 8 in the United Nations Convention on the Rights
of the Child: "The child shall be registered immediately after birth and shall have the right from
birth to a name, the right to acquire a nationality..." (CRC Article 7) and "States Parties undertake
to respect the right of the child to preserve his or her identity, including nationality, name and
family relations..." (CRC Article 8).[4]

trends such as teenage pregnancy, birth defects, and HIV. It is used to determine government
funding for public health. It also helps determine what programs get priority for government
funding such as the Abstinence and Teen Pregnancy Prevention Program, Headstart, and Children
with Special Healthcare Needs.

of purposes to assist in providing better health care for the people of Texas. The information is
compiled into categories and trends to determine what characteristics contribute to health issues
such as infant mortality and other critical infants problem.
This data can be used to implement health care programs and to determine the need for funding

We conduct this research because, this will be the easier way in getting birth certificate through
online. And to help some people and students to get their birth certificate easier. Through the
help of online application form, it will give them convenient way to get their PSA birth certificate.
To solve this problem , we will create a webpage which they can easily fill-up the form to get their
PSA birth certificate through fill upping online.

Hypothesis

HO1: There is a relationship between online PSA birth certificate form to the SHS students
of Potungan National High School .
HO2: Is there any benefits of getting PSA birth certificate online to the students?
HO3: There are possible solution in that given problem.
Research Methodology

This study attempts to answer the two research questions. (1.)What are the benefits of getting
Birth Certificate Online? (2.) How to solve this problem? This enables the researchers to identify
and explain the research problem.

Data Collection and Analysis

This study attempts to answer the research question, What are the benefits of getting a birth
certificate online?
This enables the researchers to explain through creating an online PSA birth certificate form.
The main data collection for this study was to gather information through searching on the
internet. We will using enternet explorer which is the instrument by creating a webpage. And by
running for it, we will use an internet connection to run the project.
