napagalaman ko ang pangalan ng driver ay si IRINA
ESTANISLAO na taga Villa de Calamba, La Mesa,
Calamba City.

T- Naimbistigahan ba ito ng Pulisya:

 Industrial Park Police Assistance Group na nakatalaga sa
Brgy San Jose, Sta Rosa City ang nag-imbistiga. Ito po

at sakop po ng Kasa ang repair. Nakalabas po ang sasakyan noong
Hulyo 26, 2008.

T- Bilang driver ni Dra Alma, saan ba ang regular nyong routa o

pinupuntahan?
S- Sa National Mental Hospital po sa Mandaluyong City, kung saan
siya regular na pumapasok at araw-araw ko po siya inihahatid doon
kahit sabado o lingo.

T- Habang ginagawa ang sasakyan, saan siya sumasakay papunta

S- Umarkila po siya ng sasakyan at ang bayad po ay P2,500.00
maghapon pati na po gasoline at driver.

S- Dapat pong magbayad ng danyos sina Reynaldo Endaya at Irina
Estanislao sa pagkakabangga sa menamaneho kong sasakyan na ari
ni Dra Alma Maunahan Roxas at iba pang gastos.

impleaded in her official capacity as repository of birth records within the Philippines, with
office address at Diliman, Quezon City where summons can be served.

2). That I was born on September 20, 1974 at Calamba City, Laguna, ANNEX “A”;

3) The birth was recorded under Local Civil Registry No. 1991-a-74, ANNEX “B”;

4) The error to be corrected is my birthdate “ from September 10, 1974 to

September 20, 1974” as appearing in my Certificate of Live Birth issued by Calamba Local
Civil Registry Office,

5) I am submiting the following documents to support this petition:

a) Birth Certificate issued by the Office of Civil Registrar-General;

b) Birth Certificate issued by Calamba Local Civil Registrar’s Office
c) Baptismal Certificate
d) Philippine Passport issued by San Francisco Consulate Office, CA
