LEASE AGREEMENT

KNOW ALL MEN THESE PRESENTS :

This AGREEMENT made and entered into by and between-

The _________________, an educational institution duly organized


and existing under and by virtue of laws of the Republic of the Philippines, and
the owner and operator of the _____________________, with principal
office at ___________________, represented herein by its Chief Executive Officer,
____________________ hereinafter referred to as the “LESSOR;”

and

____________________, of legal age, Filipino, and with


residential address at _____________________ hereinafter referred to as the “LESSEE.”

WITNESSETH, THAT:

WHEREAS, the LESSOR is the owner and operator of the University of Santo Tomas Hospital (the
“Hospital”). Including the Clinical Division Building whereby some portions thereof are being leased
out as clinic space for Physicians like the LESSEE. The Clinical Division Building is located at Lacson
Avenue, Sampaloc, Manila.

WHEREAS, part of the premises being leased out by the LESSOR is _______________, which has a
floor area of _________________., located at the _______________________ (the “leased
premises”):

WHEREAS, the LESSEE, represents that he/she is a duly qualified to occupy the above leased
premises and hereby accepts and binds himself/herself to strictly abide by the terms and conditions of
occupancy hereinafter provided;

WHEREAS, Rev. Fr. Roux, O.P., who is the current Chief Executive Officer of the Hospital, and was
duly authorized to represent the LESSOR in this agreement.

NOW THEREFORE, for and in consideration of the foregoing, the parties agree as follows:

1. PERIOD: This Agreement shall be for a period of ONE (1) YEAR, which shall begin on 09
September 2019 and which shall automatically end at the midnight of 09 September
2020. Renewal or extension for a similar or less period is understood to be at the sole option
of the LESSOR, the occupancy of the leased premises being a mere privilege granted by the
LESSOR. If for one (1) reason or another, no express renewal or extension in writing is made
at the time of expiry of this agreement, the occupancy of the premises shall be understood to
be on a month-to-month basis only.

The period or term as stipulated notwithstanding, the LESSEE agrees that should the
LESSOR need the subject premises for any purpose as it deems appropriate, this Agreement
may be terminated and said premises repossessed by the LESSOR, provided, however, that
the LESSOR gives a 60-day prior written notice of termination to the LESSEE.

2. GROUP OCCUPANCY FEE: For the privilege to use and occupy the leased premises, the
LESSEE shall pay to the LESSOR a monthly fee, equivalent to his pro-rata share to the total
room rental, of Seven Thousand Four Hundred Twenty-One and 87/100 Pesos Only (Php
7421.87) Philippine Currency, This amount shall become due and payable, without the
necessity of demand, within the first (5) days of each month. Payments shall be made at the
LESSOR’s Cashier Office, located at Ground Floor, Private Division Building, USTH. Without
prejudice to the LESSOR’S other options, any delayed payment shall be subject to penalty at
the rate of five percent (5%) a month until full payments is made.
