Your child wishes to participate in CFC Youth For Christ activities. These activities will take place
under the guidance and direction of CFC Couple Coordinators and volunteers of CFC Youth for
Christ. These activities will involve Christian talks, discussions, singing, games and other activities.
We expect participants to be brought and picked to and from these activities through your own
transportation. In the event that your child will be driven by somebody else, you agree to release the
driver from all or any liabilities, as a result of providing a ride thereof.
If you desire your son/daughter/individual under your guardianship to participate in CFC Youth For
Christ events or activities, please complete, sign and return this statement of consent and release
liability by the time of registration at the event location. As a parent and/or legal guardian, you
remain fully responsible for any legal responsibility, which may result from any personal actions
taken by the named participant. This consent is only valid for the youth camp stated on the other
side of this page.
Statement of Consent and Release of Liability

What is a Youth Camp?

A youth camp is a three-day retreat, usually on a weekend Friday, Saturday, and
Sunday, and conducted completely by other young adults. Here young adults
find a place where they can discover and explore the Word of God in a
supportive and caring, yet fun and exciting atmosphere. The Youth Camp
fosters new friendship as the young adults play games, entertain one another,
and share ideas and experiences in a wholesome environment where Christian
values are talked about and are actively practiced.
