AFFIDAVIT OF CONSENT

I, BARBARA TOLOP, of legal age, Filipino, resident of Cortes, Bohol,

Philippines, after having been duly sworn under oath, do hereby declar that:
1. I am the declared owner of a certain parcel of land located at Fatima, Cortes,
Bohol, Philippines, designated as Lot No. 4942, covered by Tax Declaration No.
2016-16-0004-00080;

2. By virtue of this Affidavit, A am freely giving consent to Epifanio Bitas resident

of Cortes, Bohol, to construct/build a house on the above-described parcel of
land;

3. I am executing this Affidavit to support the application of Epifanio Bitas of a

Zoning Clearance and to attest to the truth of the above statements and for
whatever legal purpose this may serve.
