AFFIDAVIT OF NON-RECEIPT
I, EDWARD CAÑEDO NAINGUE, of legal age, married, Filipino citizen and a
resident of Lot 3, Blk 3 AW Subd., Putik, Zamboanga City, Philippines after having
been duly sworn to in accordance with law, do hereby depose and state:
That I am a member of a Philippine Health Insurance Corporation;
That I am a member ever since and have been paying the
membership fee since;
That despite of my payments, I did not receive my PHIC Identification
Card;
That I exerted diligent efforts to receive and claim my PHIC; however
at the time that the PHIC card was issued, I was still employed at the Church
of Jesus Christ of the Latter Days Saints way back 2010 and my employer
never forwarded the PHIC Card to me after it was issued; and
That I am executing this affidavit to attest to the veracity of the
foregoing facts and for all legal intents and purposes this may serve.
