SUBJECT : DIVISION ORIENTATION ON VARIOUS FORMS AND

SYSTEMS FOR SY 2019-2020

1. The schedule of the DIVISION ORIENTATION ON VARIOUS FORMS AND

SYSTEMS FOR SY2019-2020 is scheduled on November 15, 2019 from 8:00 A.M.to
5:00 P.M. at the Institute of Foundational Learning Compound, Brgy San Isidro, City of
Cabuyao, Laguna.

2. The objectives of the said activity are as follows:

a. Learn and discuss the enhancements and updates on the NSBI and
LIS/EBEIS, and
b. Accomplish the NSBI Forms and EBEIS Government School Profiles for
validation of focal persons and accountable Officials.

3. Participants are Public and Private Schools LIS/EBEIS and Physical Facilities
Coordinators.
4. Attendance of all participants is a must.
5. Participants are advised to bring laptop, extension cord, and pocket wifi for internet
connectivity, and copy of the latest School Profile downloaded from LIS/EBEIS..
6. Food and other expenses of participants from the Public Schools shall be charged to
the Special Allotment Release Order (SARO) #5021408000 for the implementation of
EBEIS and LIS for SY 2019-2020.
7. Food and other expenses of participants from the Private Schools shall be handled by
the Cabuyao Private Schools Association (CAPRISA).
8. Travel expenses shall be charged against school funds subject to the usual accounting
and auditing rules and regulations.
9. Immediate and wide dissemination of this memorandum is desired.
