I, holder of Philippine Passport, would like to request the return of my passport so I can have it renewed

and go back to my home country due to the following reasons:

(1) Before my passport was confiscated, I was already planning to work and be an immigrant
in the United States of America (“USA” or “US”) as a Physical Therapist. In line with this, I
went to the US Embassy last November, 2018 and requested for a tourist visa to be able
to take the National Physical Therapy Examination (the “Exam”) in Guam, USA. I took and
passed the Exam in January, 2019 and received my US license February, 2019.

Upon passing the Exam, my agency from the US, Jackson Therapy International (“JTI”),
advised me to have my credentials assessed and evaluated by CGFNS International Inc.
(“CII”). I was further advised by JTI that CII will request for specific documents and
administer exams for CII to validate my credentials within a year, all of which are
necessary for my US immigrant visa application (“visa application”) to progress.
Correspondence with CII and the current status of my visa application are attached
herein, for your reference.

(2) I need my passport to take the Test of English as a Foreign Language (“TOEFL”), regardless
if I will take the TOEFL here in Singapore or in the Philippines. I requested for the return
of my passport in May, 2019 for this very same reason. I already booked for the TOEFL in
March, 2019, a few weeks before my passport was seized. The TOEFL requires the original
valid passport and a secondary ID of the examinee, depending on which country the
examinee is planning to take the TOEFL in. Despite of the best efforts of the Officer-in-
Charge of my case to help me take the TOEFL without my passport, the Educational
Testing Service (“ETS”) denied the request and forfeited my TOEFL registration. In this
regard, kindly take note that a valid TOEFL score is one of the requirements of CII, along
with my US and Philippine licenses, to accomplish the review of my credentials so CII can
issue a certificate in my favor stating that I am eligible for US Federal Screening.

(3) I am looking forward to enrolling in a post-professional Doctor of Physical Therapy


program (the “Doctorate Program”) offered in Philippine universities while waiting for the
approval of my visa application. Needless to say, being enrolled in a Doctorate Program
in a Philippine university will necessitate my presence in the Philippines. Moreover, in
relation to my second reason, I will need to achieve a certain valid TOEFL score to be
admitted in a Doctorate Program.

In view of all the foregoing, I humbly request that my Philippine Passport be returned to me as soon as
possible.
