DARLL JUN RAMOS CAPULOY, both Filipino citizen, of legal
ages, presently residing and with postal address at Elazequi Subd.,
Brgy, 3, Calamba City, Laguna, after having sworn according to law
depose and say:

THAT, MATHEA JHOY VIVAS CAPULOY, is our daughter;

THAT, she is due to travel to JAPAN on October to November,

2017 accompanied by her Uncle, JAY DONN CAPULOY and her
auntie VERLYN CAPULOY of Blk. 3, Lot 16 Caridad St., Villa
Remedios, Calamba City, Laguna;

THAT, MATHEA JHOY VIVAS CAPULOY is 4 years old

having been born on September 17, 2013 at Calamba City, Laguna,
hence need our parental consent to go abroad accompanied by her
uncle and auntie;

THAT, we are giving our PARENTAL CONSENT for her to travel

abroad for her wellbeing and educational benefits;

THAT, this affidavit is being executed to attest to the truthfulness

of the above facts and for whatever lawful purpose it may serve.

I, LEONORA DELOMEN KALAW, Filipino, of legal age,

married to Felecisimo Kalaw, presently residing at B 18, L 25,
Jubileeville, Brgy. Masaya, Bay, Laguna, under oath depose and
state:

THAT, I filed a case against my husband for violation of

RA 9262, VIOLENCE AGAINST WOMEN before RTC Branch
35, Calamba City;

THAT, likewise, I filed a PETITION for Issuance of

Permanent Protection Order, the TEMPORARY PROECTION
ORDER initially was issued by this Honorable Court which
expired last February 6, 2012;

THAT, I am presently sick of Hypertension and Bronchial

Asthma, Medical Certificate is attached as Annex “A” made part
of this Affidavit;

THAT, my present medical condition necessitate my

continuous use of SUSUKI VAN Plate NHO 563, for Emergency;

THAT, I am executing this affidavit to attest to the

truthfulness of the above facts and to justify my possession of a
conjugal vehicle SUSUKI VAN Plate No. NHO 563 and for any
lawful purpose it may serve.

I, MAURICIO T. SINDAY JR., Filipino citizen, of legal

age, married, presently residing at Brgy. Lawa and Brgy. Punta, both
of Calamba City, Laguna, after having sworn in accordance with law,
depose and say:

THAT, I am the oldest child of my late parents, Mauricio

Sinday and Pilar Talisaysay, of Bagobantay, Diliman, Quezon City,
born on March 28, 1952 at North General Hospital, Manila, xerox
copy of birth Certificate issued by City Hall, Manila is attached as
Annex “A” made part hereof;

THAT, my birth was registered in Manila under Register No.

3081(52);

THAT, when I secured a copy of my Birth Certificate from

NATIONAL STATISTICS OFFICE/PHILIPPINE STATISTICS
AUTHORITY, I was given a copy of my Birth Certificate Register
No. 3081 (C52) without my name appearing therein (blank), attached
as Annex “B” made part hereof;

THAT, there is a compelling urgency to correct the said

DISCREPANCY in the interest of justice and equity;

THAT, this affidavit is being executed to attest to the truthfulness

of the above facts to correct the DISCREPANCY noted in my Birth
Certificate at NSO/PSA and for whatever legal purpose it may serve.

normal mental faculties;

2. To attend Court hearing for and in my behalf with regards to the civil
aspects of the above cited criminal case;

3. To sign for and in my name transaction involving the lease and

mortgage of my real property rights pursuant to law;

4. To withdraw from my account in RCBC SAVINGS ACCOUNT NO.

1352801433 and file DISABILITY BENEFITS CLAIM with SOCIAL
SECURITY SYSTEM ID NO. 03-53658002;

5. To hire the professional services of ATTY. GREGORIO E.

MAUNAHAN, to file cases needed to seek justice and damages and
protect our legal rights under the law;

HEREBY GIVING AND GRANTING unto my said attorney-in-fact full

power and authority to do and perform every act and thing whatsoever requisite and
necessary to be done in and about the premises, as fully to all intents and purposes as I
might or could do if personally present.
