Instruction: Interview Evaluation Form is to be completed by the interviewer to rank the candidates overall qualifications for the position to which they
have applied. Under each heading, the interviewer should give the candidate a numerical rating based on the following:
5-Exceptional 4-Above Average 3-Average 2-Satisfactory 1-Unsatifactory
RATING
Prior Work Experience/Training: Has the candidate acquired relevant skills and training through
pas work experiences.
Technical Qualifications: Does the candidate have the technical skills necessary to perform the job
successfully?
Communication: How were the candidate’s communication skills during the interview (e.g. body
language, answers to questions, listening)? Does he express information and ideas in an organized
manner and responds to questions appropriately?
Enthusiasm: How much interest did the candidate show in the position and the company? Has he
shown professionalism by arriving on time and presentable? Has he taken time to know more about
the company?
Leadership Skills: Did the candidate demonstrate leadership traits and abilities?
Interpersonal Skills: Did the candidate worked in projects that involves interaction and showed the
ability to persuade during the interview?
Initiative: Did the candidate demonstrate, through their answer, a high degree of initiative? Has he
able to tell detailed scenarios where he has taken prompt action to accomplish objectives, be
proactive and achieve goals beyond what is required?
Time Management: Did the candidate demonstrate, through his answer, show the ability to become
aware on how he us his time in an organized, efficient and effective ways at work?
Customer Service: Did the candidate demonstrate a high level of customer service skills?
