maintenance of Barangay Motor vehicle had been purchased through
emergency mode of procurement;

2. That the aforementioned fuel was supplied and delivered by various suppliers
covered by corresponding official receipts/cash invoices with a total amount of
(P 2,000.00);

3. That said emergency purchase has been resorted to in order to hasten the
official activity/ies of the Barangay which, if not done, may disrupt the smooth
and continuous delivery of basic services;

4. That the amount has been duly appropriated and obligated and fund for the
purpose is available;

5. That the prices contracted were found to be the lowest obtainable in the
market at the time of the purchase;

6. That due to the urgency in need, there was no time to procure said supplies
through the regular mode of procurement.
