EXCERPT FROM THE MINUTES OF THE SPECIAL MEETING HELD
AT THE DRAFTING ROOM ON JULY 30, 2019 AT 9:30 – 9:45 A.M.

WHEREAS, the school conducted a “DINNER FOR A CAUSE” on March 9, 2019 and was able
to raise a cash proceed amounting to One hundred three thousand, fifty-one pesos (Php
103,051.00);

WHEREAS, the amount was entrusted to the Faculty and Employees Club and was deposited
under the names of Mrs. Lewalyn L. Constantino (FEC President) and Mr. Roberto S. Godoy
(FEC Treasurer) at Philippine National Bank, Laoag City;

WHEREAS, the said amount will be withdrawn to be used in putting-up of a water refilling station
of the school;

BE IT FINALLY RESOLVED that the amount of One hundred three thousand pesos
(Php 103,000.00) be withdrawn by the signatories immediately after the approval of the club
adviser.

RESOLVED FURTHER that this resolution be submitted to the club adviser for approval.

Mrs. Leah M. Visconde (FEC Assistant Secretary) moved for the withdrawal of the said amount
and was unanimously seconded by the body.
