Terms and conditions:This agreement is effective 12:01 AM local time on the

effective date (date signed), and for the period indicated. Prices are subject to
change after completion of the initial term indicated. Annual visits will be
performed during normal business hours, and under the schedule of Mister Quik (MQ).
Plans will auto renew for 1 year periods upon completion of the initial term.
Cancellations require a letter sent to MQ or an e-mail sent to
cancel@misterquik.com, within 15 days of the renewal date in order to avoid
charges. All monies paid are non-refundable. All promotions, offerings, discounts,
and anything else stated are valid only while account is paid and in good
standings. Any balances from maintenance visits already received will be due at the
time of cancellation. Agreements will become void with a past due account (over 1
day). I agree to term for at least 90 days. The visits / tune ups have no monetary
value, and are not a contractual component to this agreement. (1) Extended
Guarantee(s) ONLY covers the work previously performed by MQ as a malfunction of
its installation and or equipment. It does not cover normal wear and tear,
previously or existing conditions. Dispatch fees and values vary. (2) Subscriber
will pay an emergency fee for ELECTRICAL, HVAC & PLUMBING SERVICE work for as long
as they are a plan member. (3) HVAC, Plumbing and Electrical visits are not a
contractual component to this agreement. MQ will perform visits as their schedule
allows, or at the request of the customer.Tune Ups / Visits have no monetary value,
and are not a contractual component to this agreement. All final request for visits
must be made in writing to Mister Quik to 4625 W. 86th Street, Suite 800,
Indianapolis, IN 46268. All visits expire and terminate after agreement period is
complete. (4) 100% Money Back Guarantee. All money back guarantee requests applying
to SHAPE plan must be submitted in writing within 90 Days of start date. (5) Terms
and conditions subject to change with out prior notice.
