The physical dimension of wellness involves encouraging regular activities that produce endurance,
flexibility, and strength. Read each statement carefully and respond honestly by using the following
scoring:

Almost always = 2 points Sometimes/Occasionally = 1 point Very Seldom = 0 point

__0__1. I exercise aerobically (vigorous, continuous) for 20 to 30 minutes at least three times per week.

__1__4. I wear a seat belt while riding in or driving a car.

__1__5. I deliberately minimize my intake of cholesterol, dietary fats and oil.

__1__6. I avoid drinking alcoholic beverages or I consume no more than one drink per day.

__2__7. I get an adequate amount of sleep.

__2__8. I have adequate coping mechanisms for dealing with stress.

__1__9. I maintain a regular schedule of immunizations, physical, dental check-ups and self exams.

__2__10. I maintain a reasonable weight, avoiding extremes of overweight and underweight.

CONTENT I have learned about some basic self defense mechanisms
that will help me defend myself form harm.

REASONS It is because violence is rampant. Self defense will help us
protect ourselves

PHYSICAL DEVELOPMENT As a student, I am required to attend the class. I am
somewhat knowledgeable and I enjoyed every lesson about

What does learning experience mean to me?
PERSONAL DEVEOPEMENT As an adult, I myself may be a victim of violence, we may
never know. But being equipped with the basic self defense
