Process to Apply
1- Click on the link: www.axisbank.com/mydesign

2- Enter first 8 digits of your Debit card number and accept the terms & conditions as shown in the
screen shot below. Click on Submit

3- Upon being routed to the next page, choose “International” Debit Card under usage preference
and click proceed.

It must be noted that NRO customers are issued Domestic Cards only. They should choose
“Domestic” Debit Card

4- You will be routed to the page for uploading the image. Please wait till the page is loaded

5- You may upload image of your choice* or you may choose from an array of gallery images as
shown in the screens below

6- You may use options like flip / rotate /move the image as per the requirement and click Next
once you finish editing.

Please note:- As part of uploading process, the size of image, format etc. shall be verified and
in case, if the same does not confirm to the requirements, you shall be informed to upload
another image

7- You will be able to see the debit card preview in the next screen. If the same is not as per your
expectation, you may click Back button to upload a new image and then click Next again, to
preview the card with the new image.Click on ‘Submit’ button if the card preview is as per your
expectation.

8- You will be re-directed on the internet banking login page for payment. Enter your login id and
password to proceed

9- Please check the account details as shown by clicking on the option as shown below

10- You will now see page showing details against your account as shown below. Please select the
‘type of card’ from the drop down list shown in the link. Fill the name desired on card section
and click on Pay button to proceed further. You will receive a NetSecure Code for one time
authentication through SMS / IVR / Email.

Important point: Please note down the imge code and use the same for any future reference/
communication with Bank pertaining to your MyDesign Debit Card application

11- Enter the NetSecure code (received through SMS / IVR / Email) in the section as shown in the
screen below and click on Confirm button. An initial debit of Rs. 1 shall be levied towards
internet handling charges.

Please note this is not your Debit Card issuance fee. You will be charged replacement fee as per
your card variant and Rs. 150+taxes towards your new MyDesign card once the card is issued.

12- You will now be able to see the confirmation page for the request

 Important Notes:

a) Please ensure to note down the image code for any future references with respect to your
MyDesign Debit Card application

b) After successful application scrutiny, the card printing and processing is done and the
MyDesign Debit Card shall be sent to customer address registered with Axis Bank

c) The content mentioned here is proprietary of Axis Bank. Any duplication or misrepresentation
of this shall be deemed as a legal offence

d) *Image uploaded by the customer should be as per the Terms & Conditions as defined
