SCHENGEN VISA INFORMATION

Prepare all the necessary documents before you personally deliver the
application to the Embassy, otherwise the application might be returned

All documents have to be translated into English or Slovak. If necessary, the

verification should be made by the relevant authorities.

1. Schengen visa application form, completed and signed.

2. Travel document – passport – Passport/Travel document shall have been issued
within the previous 10 years & should be valid for 90 days after expiry of the visa with
at least two blank pages
3. Passport photo (white background) not older than six months (35-40 mm)

5. Proof of the official stay in Cyprus (“Pink Slip”)

TOURIST PURPOSE:
- Original of hotel voucher  with confirmation of payment for stay in Slovakia
 Condition of voucher
Name and address of travel agency, registration number of voucher, first and last
name of hotel guest, date of arrival and departure, description of hotel services,
confirmation of the authorized person of the travel agency. If the voucher does not
include the confirmation of payment, applicant has to submit separate invoice with
the number, stamp and signature of bank statement confirming the payment on the
account of hotel / agency.

FAMILY/FRIEND VISIT:
- official invitation form verified by the Slovak Foreign Police Department
- original invitation letter of guarantee for all expenses from the person in
Slovakia legalized by a notary public and/or financial guarantee from the
applicant
- documentation of relationship if family visit
- copy of the reference's residence permit (if the reference person is not a
Slovak citizen)

BUSINESS PURPOSE:
- official invitation form from Slovak company/business partner verified by the
Slovak Foreign Police Department
- invitation letter from the Slovak business partner legalized by a notary public,
stating the purpose, duration and itinerary for the whole stay in Schengen
area
- original letter from your company stating reason for your stay in Slovakia
- proof of your current occupation
- confirmation of accommodation in Slovakia
CONFERENCES:
- official invitation form from Slovak company verified by the Slovak Foreign Police
Department
- invitation letter from company in Slovakia legalized by a notary public

- proof of your current occupation, original letter from your company stating a reason
for your stay in Slovakia

- confirmation of accommodation

Note: In the case that you have never been in the territory of Schengen area it is
mandatory to submit the official invitation form verified by Slovak Foreign Police
Department.

5. Document confirming financial resources for covering costs of your stay in

Slovakia and return journey:
- documentation of own funds for the stay or a guarantee declaration issued by the
reference person financial means (in invitation, or in guarantee letter etc.)
- original of your bank account(s) for the last 6 months
- original of the salary slip for the last six months
- Traveller´s Cheques, Credit Card statement for the last six months, etc.

6. Confirmed booking (travel itinerary) of roundtrip air ticket, with fixed travel dates
of maximum 90 days

Do not buy the ticket until visa has been granted.

If the application is approved, the visa will be issued according to the dates on the
booking.

7. Valid medical travel insurance for the entire journey in the Schengen area with
minimum coverage of EURO 30.000.

8. All applicants under 18 years old can be ask to submit:

- If you are travelling alone: An original of letter of consent signed by both
parents and legalized.
- If you are travelling with only one of your parents: An original of letter of
consent from the other parent and legalized.
- If the parent you are travelling with has single custody: A shown original of
certificate of single custody.
- Copies of the passport of the parents and/or birth certificate of applicant
and copies of ID cards of the parents
The Embassy may always request additional documents.

The application fee must be paid at the Embassy in the day of the appointment.

National Visa (with granted residence permit) 9.50 EUR

All applicants have to come for personal interview.

The processing time for visa is up to ten working days from the day the
Embassy receives the application in written form with all the documents and the fee
paid.
