ﻤﻘﺩﻤـﺔ
ﻓﺘﺭﺓ ﺍﻻﻤﺘﻴﺎﺯ ﻤﻥ ﺃﻫﻡ ﺍﻝﻔﺘﺭﺍﺕ ﻓﻲ ﺍﻝﻤﺴﻴﺭﺓ ﺍﻝﻤﻬﻨﻴﺔ ﻝﻠﻁﺒﻴﺏ ،ﻓﻬﻲ ﻤﺭﺤﻠﺔ ﺘﺤﻭﻴل ﺍﻝﻤﻌﻠﻭﻤﺎﺕ ﺍﻝﻨﻅﺭﻴﺔ ﺇﻝﻰ ﺘﻁﺒﻴﻘﻴﺔ ،ﻭﻫﻲ
ﻼ ﻋﻥ
ﻤﺭﺤﻠﺔ ﻜﺴﺏ ﺍﻝﻜﺜﻴﺭ ﻤﻥ ﺍﻝﻤﻬﺎﺭﺍﺕ ﺍﻝﻁﺒﻴﺔ ،ﻭﻤﻬﺎﺭﺍﺕ ﺍﻻﺘﺼﺎل ﻭﺍﻝﺘﻭﺍﺼل ﻤﻊ ﺍﻝﻤﺭﻀﻰ ﻭﻤﻊ ﺯﻤﻼﺀ ﻭﺍﻝﻌﻤل ،ﻓﻀ ﹰ
ﻜﻭﻨﻬﺎ ﺍﻝﻔﺘﺭﺓ ﺍﻝﺘﻘﻠﻴﺩﻴﺔ ﺍﻝﺘﻲ ﻴﺴﺘﻁﻴﻊ ﻁﺒﻴﺏ ﺍﻻﻤﺘﻴﺎﺯ ﻓﻴﻬﺎ ﺍﻝﺴﺅﺍل ﻋﻥ ﻤﺎ ﻴﺨﻁﺭ ﺒﺫﻫﻨﺔ ﻓﻬﻭ ﻤﺎﻴﺯﺍل ﺘﺤﺕ ﺍﻝﺘﺩﺭﻴﺏ،
ﻭﻤﺴﺌﻭﻝﻴﺎﺘﻪ ﻤﺤﺩﻭﺩﺓ ،ﻭﺤﻭﻝﻪ ﺍﻝﻜﺜﻴﺭ ﻤﻥ ﺍﻻﺴﺘﺸﺎﺭﻴﻴﻥ ﻭﺍﻻﺨﺘﺼﺎﺼﻴﻴﻥ ﻭﺃﻁﺒﺎﺀ ﺍﻝﺩﺭﺍﺴﺎﺕ ﺍﻝﻌﻠﻴﺎ ،ﻭﻤﻌﻅﻤﻬﻡ ﺠﺎﻫﺯﻭﻥ
ﻝﺘﻭﺠﻴﻬﻪ ﻭﻤﺴﺎﻨﺩﺘﻪ ﻭﺇﺭﺸﺎﺩﻩ ،ﻜﻤﺎ ﺃﻥ ﻓﺘﺭﺓ ﺍﻻﻤﺘﻴﺎﺯ ﻝﻬﺎ ﺃﺜﺭ ﻜﺒﻴﺭ ﻓﻲ ﺘﺤﺩﻴﺩ ﻤﺠﺎل ﺍﻻﺨﺘﺼﺎﺹ ﻻﺤﻘﺎﹰ ،ﻭﻋﺎﺩﺓ ﻤﺎ ﺘﺼﺎﻍ
ﺤ ﹺﺭﻴﺔ ﺒﺎﻻﻫﺘﻤﺎﻡ.
ﻓﻴﻬﺎ ﺸﺨﺼﻴﺔ ﺍﻝﻁﺒﻴﺏ ﻭﻴﺒﺩﺃ ﺘﻁﺒﻴﻘﻪ ﺍﻝﻔﻌﻠﻲ ﻷﺨﻼﻗﻴﺎﺕ ﺍﻝﻤﻬﻨﺔ ﺍﻝﻁﺒﻴﺔ ،ﻭﻜل ﺫﻝﻙ ﻴﺠﻌﻠﻬﺎ ﻓﺘﺭ ﹰﺓ
ﺃ -ﺘﻌﺭﻴﻑ ﻓﺘﺭﺓ ﺍﻻﻤﺘﻴﺎﺯ :ﻫﻲ ﻓﺘﺭﺓ ﺍﻝﺘﺩﺭﻴﺏ ﺍﻝﺘﻁﺒﻴﻘﻲ ﺍﻹﻝﺯﺍﻤﻴﺔ ﻝﻠﺨﺭﻴﺠﻴﻥ ﺍﻝﺤﺎﺼﻠﻴﻥ ﻋﻠﻰ ﺒﻜﺎﻝﻭﺭﻴﻭﺱ ﺍﻝﻁﺏ ﻭﺍﻝﺠﺭﺍﺤﺔ
)ﺍﻝﺫﻴﻥ ﺃﻨﻬﻭﺍ ﺠﻤﻴﻊ ﻤﺘﻁﻠﺒﺎﺕ ﺍﻝﺘﺨﺭﺝ ﺒﻨﺠﺎﺡ( ،ﻭﺘﺴﻤﻰ ﺴﻨﺔ ﺍﻻﻤﺘﻴﺎﺯ ،ﻭﻤﺩﺘﻬﺎ 12ﺸـﻬﺭﺍﹰ ،ﻭ ﹸﺘﻘـﻀﻰ ﻓـﻲ ﺍﻝﺘـﺩﺭﻴﺏ
ﻼ ﻝﻠﺠﻠﻭﺱ ﻻﻤﺘﺤﺎﻥ ﻤﺯﺍﻭﻝﺔ ﻤﻬﻨﺔ ﺍﻝﻁﺏ ﺇﻻ ﺒﻌﺩ ﺇﺘﻤﺎﻤﻪ ﺘﻠﻙ ﺍﻝﻔﺘﺭﺓ ﺒﻨﺠﺎﺡ.
ﺍﻹﻜﻠﻴﻨﻴﻜﻲ ﺍﻝﻤﻴﺩﺍﻨﻲ ﻭﻻ ﻴﻌﺘﺒﺭ ﺍﻝﺨﺭﻴﺞ ﻤﺅﻫ ﹰ
ﺍﻻﻝﺘﺯﺍﻡ ﺒﻨﻅﺎﻡ ﺍﻝﺨﺩﻤﺔ ﺍﻝﻤﺩﻨﻴﺔ )ﺍﻝﻤﺘﻌﻠﻕ ﺒﺤﻘﻭﻗﻪ ﻭﻭﺍﺠﺒﺎﺘﻪ ﻭﺍﻝﺘﺯﺍﻤﻪ ﺍﻝﻤﻬﻨﻲ ﻭﺍﻝﻭﻅﻴﻔﻲ ﻭﺍﻝﺴﻠﻭﻜﻲ(. .1
ﺍﻻﻝﺘﺯﺍﻡ ﺒﺄﻭﻗﺎﺕ ﺍﻝﺩﻭﺍﻡ ﻭﺘﻌﻠﻴﻤﺎﺕ ﺍﻝﺤﻀﻭﺭ ﻭﺍﻻﻨﺼﺭﺍﻑ. .2
ﺍﻻﻝﺘﺯﺍﻡ ﺒﺎﻝﺒﺭﻨﺎﻤﺞ ﺍﻝﺘﺩﺭﻴﺒﻲ. .3
ﺍﻝﻭﺍﺠﺒﺎﺕ ﺍﻝﻔﻨﻴﺔ:
ﻴﻘﻭﻡ ﻜل ﻗﺴﻡ ﺒﺘﺤﺩﻴﺩ ﺍﻝﻤﻬﺎﻡ ﺍﻝﻁﺒﻴﺔ ﺍﻝﺘﻲ ﻴﺠﺏ ﻋﻠﻰ ﻁﺒﻴﺏ ﺍﻻﻤﺘﻴﺎﺯ ﺃﻥ ﻴﻤﺎﺭﺴﻬﺎ ﺨﻼل ﻓﺘﺭﺓ ﺘﺩﺭﻴﺒﻪ ﻓﻲ ﺍﻝﻘﺴﻡ ﻭﻫﻲ ﺠﺯﺀ
ﻻ ﻴﺘﺠﺯﺃ ﻤﻥ ﻫﺫﺍ ﺍﻝﺘﻭﺼﻴﻑ:
ﻴﻘﻭﻡ ﻁﺒﻴﺏ ﺍﻻﻤﺘﻴﺎﺯ ﺒﺎﻝﻜﺸﻑ ﺍﻷﻭﻝﻰ ﻋﻠﻰ ﺍﻝﻤﺭﻴﺽ ﻭﺃﺨﺫ ﺍﻝﺘﺎﺭﻴﺦ ﺍﻝﻤﺭﻀﻲ ،ﻭﺘﺴﺠﻴل ﺍﻝﻌﻼﻤﺎﺕ ﺍﻝﻤﺭﻀﻴﺔ ﻝﻜـل .1
ﻤﺭﻴﺽ ﺘﺤﺕ ﺭﻋﺎﻴﺘﻪ ﻭﺘﻭﺜﻴﻕ ﺫﻝﻙ ﺒﻤﻠﻑ ﺍﻝﻤﺭﻴﺽ ،ﻜﻤﺎ ﺃﻥ ﻋﻠﻴﻪ ﺘﺴﺠﻴل ﺍﻝﺘﺸﺨﻴﺹ ﺍﻝﻤﺒﺩﺌﻲ ﻭﺍﻗﺘﺭﺍﺡ ﺍﻝﻔﺤﻭﺼﺎﺕ
ﺍﻷﻭﻝﻴﺔ ﻭﺨﻁﺔ ﺍﻝﻌﻼﺝ ﻋﻠﻰ ﺃﻥ ﻻ ﻴﺒﺩﺃ ﻓﻲ ﺘﻨﻔﻴﺫ ﺨﻁﺔ ﺍﻝﻌﻼﺝ ﺇﻻ ﺒﻌﺩ ﺇﻗﺭﺍﺭﻫﺎ ﻤﻥ ﻗﺒـل ﺃﺤـﺩ ﺃﻁﺒـﺎﺀ ﺍﻝﻔﺭﻴـﻕ
ﺍﻝﻤﺴﺅﻭل ﺒﺎﻝﻘﺴﻡ ﻭﻋﻠﻰ ﻁﺒﻴﺏ ﺍﻻﻤﺘﻴﺎﺯ ﻤﺘﺎﺒﻌﺔ ﺘﻨﻔﻴﺫ ﺍﻝﻌﻼﺝ ﺍﻝﻤﻭﺼﻭﻑ ﻭﻨﺘﺎﺌﺞ ﺍﻝﻔﺤﻭﺼﺎﺕ ﺍﻝﻤﻁﻠﻭﺒﺔ ﻝﻠﻤـﺭﻴﺽ
ﻭﻤﺭﺍﻗﺒﺔ ﺃﻱ ﺘﻁﻭﺭﺍﺕ ﻤﺭﻀﻴﺔ ﺠﺩﻴﺩﺓ ﺘﻁﺭﺃ ﻋﻠﻰ ﺤﺎﻝﺔ ﺍﻝﻤﺭﻴﺽ ﻭﺘﻭﺜﻴﻘﻬﺎ ﻓﻲ ﻤﻠﻔﻪ) .ﺇﻻ ﻓﻲ ﺤﺎل ﻁﻠﺏ ﺍﻝﻁﺒﻴـﺏ
ﺍﻝﻤﺸﺭﻑ ﺨﻼﻑ ﺫﻝﻙ(.
ﻴﻘﻭﻡ ﺒﺘﺩﻭﻴﻥ ﺠﻤﻴﻊ ﺍﻝﻤﻼﺤﻅﺎﺕ ﻭﺍﻝﻤﺘﺎﺒﻌﺔ ﺍﻝﻴﻭﻤﻴﺔ ﻝﻠﻤﺭﻀﻰ ﺩﺍﺨل ﺍﻷﻗﺴﺎﻡ ﺍﻝﻤﻜﻠﻑ ﺒﻬﺎ ،ﻭﻴﻜﻭﻥ ﻋﻠﻰ ﻋﻠﻡ ﺘﺎﻡ ﻋﻥ .2

ﺤﺎﻝﺔ ﺠﻤﻴﻊ ﺍﻝﻤﺭﻀﻰ ﻓﻲ ﻗﺴﻤﻪ ﻭﻋﻥ ﺘﻁﻭﺭﺍﺕ ﻭﻀﻊ ﻜ ٍ
ﻴﺴﺘﺠﺩ ﻤﻥ ﺃﻭﻀﺎﻋﻬﻡ.
ﺘﻌﺒﺌﺔ ﻨﻤﺎﺫﺝ ﻁﻠﺒﺎﺕ ﺍﻝﻔﺤﻭﺹ ﺍﻝﻤﺨﺒﺭﻴﺔ ﻭﺍﻝﺸﻌﺎﻋﻴﺔ ﺤﺴﺏ ﻤﺎ ﻴﻔﻭﻀﻪ ﺍﻝﻁﺒﻴﺏ ﺍﻝﻤﺸﺭﻑ ﻭﺘﺤﺕ ﻤﺴﺅﻭﻝﻴﺘﻪ. .3
ﻴﻠﺘﺯﻡ ﺒﺤﻀﻭﺭ ﺍﻝﺠﻭﻻﺕ ﺍﻝﺴﺭﻴﺭﻴﺔ ﺍﻝﺘﻲ ﻴﻘﻭﻡ ﺒﻬﺎ ﺃﻁﺒﺎﺀ ﺍﻝﻘﺴﻡ ﻭﻴﻨﻔﺫ ﺘﻌﻠﻴﻤﺎﺘﻬﻡ ،ﻜﻤﺎ ﺃﻨﻪ ﻋﻠﻴـﻪ ﺍﻝﻤـﺸﺎﺭﻜﺔ ﻓـﻲ .4
ﺍﻷﻨﺸﻁﺔ ﺍﻝﻌﻠﻤﻴﺔ ﺒﺎﻝﻘﺴﻡ.
ﻴﻘﻭﻡ ﺒﺎﻝﺘﺤﻘﻕ ﻤﻥ ﺴﻼﻤﺔ ﺘﻨﺎﻭل ﺍﻝﻤﺭﻴﺽ ﻝﻠﻌﻼﺝ ﺩﺍﺨل ﺍﻝﻤﺴﺘﺸﻔﻲ ﻭﻴﺨﺒﺭ ﺍﻝﻁﺒﻴﺏ ﺍﻝﻤﺴﺌﻭل ﻋﻨﻪ ﻤﺒﺎﺸﺭﺓ ﻋـﻥ ﺃﻱ .5
ﻤﺨﺎﻝﻔﺔ ﺃﻭ ﻤﻼﺤﻅﺔ ﺨﺎﺼﺔ ﺒﺘﻨﺎﻭل ﺍﻝﻌﻼﺝ.
ﻴﺭﺍﻓﻕ ﺍﻝﻤﺭﻀﻰ ﻷﻗﺴﺎﻡ ﺍﻝﻔﺤﻭﺼﺎﺕ ﺍﻝﻁﺒﻴﺔ ﺍﻷﺨﺭﻯ )ﺍﻷﺸﻌﺔ ،ﺍﻝﻤﻨﺎﻅﻴﺭ ﺍﻝﺘﺸﺨﻴﺼﻴﺔ ،ﺍﻝﺘﺤﻭﻴل ﻝﻤﺴﺘﺸﻔﻰ ﺁﺨـﺭ، .6

• ﻁﺒﻴﺏ ﺍﻻﻤﺘﻴﺎﺯ ﻏﻴﺭ ﻤﺴﺌﻭل ﻋﻥ ﺇﺨﺭﺍﺝ ﺍﻝﻤﺭﻴﺽ ﻤﻥ ﺍﻝﻤﺴﺘﺸﻔﻰ ﻜﻤﺎ ﻻ ﻴﻜﺘﺏ ﺍﻝﻭﺼﻔﺎﺕ ﺍﻝﻁﺒﻴﺔ ﺃﻭ ﺍﻝﺘﻘـﺎﺭﻴﺭ
ﺍﻝﻘﻀﺎﺌﻴﺔ ﻭﻻ ﻴﻌﻁﻲ ﺍﻹﺠﺎﺯﺍﺕ ﺍﻝﻤﺭﻀﻴﺔ.
ﺍﻝﻭﺍﺠﺒﺎﺕ ﺍﻹﺩﺍﺭﻴﺔ:
ﻴﻠﺘﺯﻡ ﺒﺎﺭﺘﺩﺍﺀ ﺍﻝﺯﻱ ﺍﻝﺭﺴﻤﻲ )ﺍﻝﺒﺎﻝﻁﻭ ﺍﻝﻁﺒﻲ( ﻭﻭﻀﻊ ﺒﻁﺎﻗﺔ ﺍﻝﺘﻌﺭﻴﻑ ﻓﻲ ﺠﻤﻴﻊ ﺃﻤﺎﻜﻥ ﺍﻝﺘﺩﺭﻴﺏ. .1
ﻴﻘﻭﻡ ﻜل ﻁﺒﻴﺏ ﻤﺘﺩﺭﺏ ﺒﺘﻌﺒﺌﺔ ﺍﻝﻨﻤﺎﺫﺝ ﺍﻝﻤﺘﻌﻠﻘﺔ ﺒﺎﻝﺘﺩﺭﻴﺏ ﺍﻝﻴﻭﻤﻲ ﻭﺘﻭﻗﻴﻌﻬﺎ ﻤﻥ ﺍﻝﻁﺒﻴﺏ ﺍﻝﻤﺸﺭﻑ ﻓﻲ ﺍﻝﻘﺴﻡ ﻭﻤﻥ .2
ﺜﻡ ﺘﻭﻗﻴﻊ ﺍﻝﻨﻤﻭﺫﺝ ﻓﻲ ﻨﻬﺎﻴﺔ ﻜل ﻓﺘﺭﺓ ﺘﺩﺭﻴﺒﻴﺔ ﻭﺘﺴﻠﻴﻡ ﺍﻝﻨﻤﺎﺫﺝ ﺇﻝﻰ ﻤﻨﺴﻕ ﺍﻝﺒﺭﻨﺎﻤﺞ ﺍﻝﺫﻱ ﻴﻘﻭﻡ ﺒﺭﻓﻌﻬـﺎ )ﺸـﻬﺭﻴﹰﺎ(

ﻤﻼﺤﻅﺎﺕ: ﺹ-
(1ﻻ ﻴﺴﻤﺢ ﺒﺘﻜﺭﺍﺭ ﺴﻨﺔ ﺍﻻﻤﺘﻴﺎﺯ ﺴﻭﺍ ﺀ ﻝﻤﻥ ﻗﻀﺎﻫﺎ ﺩﺍﺨل ﻓﻠﺴﻁﻴﻥ ﺃﻭ ﺨﺎﺭﺠﻬﺎ ،ﻭﻜﺫﻝﻙ ﺍﻷﻤﺭ ﺒﺎﻝﻨﺴﺒﺔ ﻝﻠﻔﺌﺎﺕ ﺍﻝﻤﻌﻔﺎﺓ

(2ﻴﺤﺼل ﺍﻝﻁﺒﻴﺏ ﺍﻝﺫﻱ ﺍﺠﺘﺎﺯ ﺘﻘﻴﻴﻡ ﺍﻝﻘﺴﻡ ﺒﻨﺠﺎﺡ ﻭﺒﻤﻌﺩل %90ﻭﻤﺎ ﻓﻭﻕ ﻋﻠﻰ ﻜﺘﺎﺏ )ﺸﻜﺭ ﻭﺘﻘﺩﻴﺭ( ﺒﻌـﺩ ﺇﻨﻬـﺎﺀ
ﺴﻨﺔ ﺍﻻﻤﺘﻴﺎﺯ ﻜﺎﻤﻠﺔ ﺘﺘﻀﻤﻥ ﻓﻴﻬﺎ ﺍﻷﻗﺴﺎﻡ ﺍﻝﺘﻲ ﺤﺼل ﻓﻴﻬﺎ ﻋﻠﻰ ﺫﻝﻙ ﺍﻝﻤﻌﺩل.
(3ﻴﺘﻡ ﺍﺤﺘﺴﺎﺏ ﻓﺘﺭﺍﺕ ﺘﺩﺭﻴﺏ ﺍﻻﻤﺘﻴﺎﺯ ﺍﻝﺨﺎﺭﺠﻴﺔ ﺤﺎل ﻜﺎﻨﺕ ﻤﺘﻭﺍﻓﻘﺔ ﻤﻊ ﻤﺴﺎﻗﺎﺕ ﺍﻝﺒﺭﻨﺎﻤﺞ ﺍﻝﻤﻌﺘﻤﺩﺓ ،ﻤﻊ ﺇﺤـﻀﺎﺭ
ﺍﻝﻤﺴﺘﻨﺩﺍﺕ ﺍﻝﺘﻲ ﺘﻭﻀﺢ ﺫﻝﻙ ﻤﺼﺩﻗ ﹰﺔ ﺤﺴﺏ ﺍﻷﺼﻭل.
(4ﺍﻝﻤﺴﺘﻨﺩﺍﺕ ﺍﻝﺼﺎﺩﺭﺓ ﻤﻥ ﻓﻠﺴﻁﻴﻥ ) ﺍﻝﺸﻬﺎﺩﺍﺕ ﻭﻜﺸﻭﻑ ﺍﻝﺩﺭﺠﺎﺕ "ﺍﻝﺜﺎﻨﻭﻴﺔ ﺍﻝﻌﺎﻤﺔ ﻭﺍﻝﺠﺎﻤﻌـﺔ ﻭﺍﻻﺨﺘـﺼﺎﺹ"(
ﻴﺘﻭﺠﺏ ﺃﻥ ﺘﻜﻭﻥ ﻤﺼﺩﻗﺔ ﻤﻥ ﺍﻝﺘﻌﻠﻴﻡ ﺍﻝﻌﺎﻝﻲ ﺍﻝﻔﻠﺴﻁﻴﻨﻲ ،ﻭﺍﻝﻤﺴﺘﻨﺩﺍﺕ ﺍﻝﺼﺎﺩﺭﺓ ﻤﻥ ﺨﺎﺭﺝ ﻓﻠﺴﻁﻴﻥ ﻴﺘﻭﺠـﺏ ﺃﻥ
ﺘﻜﻭﻥ ﻤﺼﺩﻗﺔ ﻤﻥ ﻭﺯﺍﺭﺓ ﺍﻝﺨﺎﺭﺠﻴﺔ ﻭﻤﻥ ﺍﻝﺘﻌﻠﻴﻡ ﺍﻝﻌﺎﻝﻲ ﺍﻝﻔﻠﺴﻁﻴﻨﻲ ﻭﺫﻝﻙ ﻝﻠﺸﻬﺎﺩﺍﺕ ﻭﻜﺸﻭﻑ ﺍﻝﺩﺭﺠﺎﺕ )ﺍﻝﺜﺎﻨﻭﻴﺔ
ﺍﻝﻌﺎﻤﺔ ﻭﺍﻝﺠﺎﻤﻌﺔ ﻭﺍﻻﺨﺘﺼﺎﺹ( ،ﻭﻓﻘﻁ ﻤﻥ ﻭﺯﺍﺭﺓ ﺍﻝﺨﺎﺭﺠﻴﺔ ﺒﺎﻝﻨﺴﺒﺔ ﻝﻔﺘﺭﺍﺕ ﺍﻝﺨﺒﺭﺓ ﻭﺍﻻﻤﺘﻴﺎﺯ.
