AFFIDAVIT OF UNDERTAKING

I, _____________________, of legal age, filipino, married, a

resident of ______________________, ___________ Philippines, after
having duly sworn to in accordance with law, do hereby depose
and state that:

1. I am the operations manager of

______________________________ (the “Company”), a
corporation duly organized and existing under and by
virtue of Philippine laws with principal place of business
at ___________________________________;

2. The Company is the contractor of

___________________________ (the “Project”) located at
______________________;

3. The Project had been completed in ____________ as per

________________ submitted to the _____________________. In
fact, the ____________________ has been using the Project.
However, up to present the Certificate of Completion for
the Project is not yet issued by the ______________________
in favor of the Company;

4. Recently, the ________________ has requested some

rectification works on the Project which the Company
signifies its willingness to conduct the requested repairs
as warranty works;

5. For the issuance of the Certificate of Completion, the

Company hereby undertakes to conduct the warranty
works as listed by the _______________________, hereto
attached as Annex-A, including those items which are not
on the Plans and Specifications (the “Scope of Warranty
Works”);
 6. In addition, the Company undertakes to complete the
warranty works on or before January 31, __________;

7. I am executing this Affidavit of Undertaking in my

capacity as operations manager and representative of the
Company;

8. Furthermore, I am executing this affidavit to attest to the

truth of the foregoing statements.
