Brgy. Pulong Buhangin, Sta. Maria, Bulacan, 3022, Philippines.

I am Millard De Vera Peñamante, holder of Wildlife Farm Permit (WFP) No. III-
2018-07, issued by the Regional Executive Director of the Department of Environment and
Natural Resources (DENR), Region 3, City of San Fernando, Pampanga on September 11,
2018, valid until CY 2023.

My experiences in the aviary business/farm operation have started since CY 2002 up

to present.

Should this merit your kind consideration, I shall abide by your choice of date of
inspection at your most convenient time.

Birds Я Us Aviary facilities and stocks of different bird species, registered under my name,
covered with Wildlife Farm Permit (WFP) No. III-2018-07, issued by the Regional Executive
Director, DENR Region 3, City of San Fernando, Pampanga, on September 11, 2018, located
at Km. 37, Pulong Buhangin, Sta. Maria, Bulacan, Philippines, before the office of the
Secretary Ministry of Agriculture, Director General of Livestock and Animal Health
Services, Jakarta, Indonesia.

In view of this, may I ask for assistance from your office, if any, to help me qualify
for the said purpose?

species, to wit:

I intend to import the aforesaid animal species from accredited breeder/exporter, Iwan
Mister Brugpas 25 5814AN Veulen, The Netherlands, which I proposed to keep and develop
in my farm lot located at Km. 37, Brgy. Pulong Buhnagin, Sta. Maria, Bulacan, Philippines.

Attached is the Veterinary Certificate issued by the Country of origin and result of
blood tests conducted on the said animals by the WAGENINGEN University and Research,
The Netherlands, dated October 8, 2018, for your reference.
