1.0 Purpose
• The Behavior Based Safety Management Program is a proactive element of
Ardent’s HS&E Management System. Ardent believes the key to eliminating
workplace incidents, injuries, and illnesses, is to modify employee behavior -
reducing unsafe acts and conditions and increasing safe work practices and
habits. Ardent objective is to pursue this goal by observing employees as they
work and talk with them to encourage safe work practices and eliminate at-risk
behaviors.

2.0 Policy
2.1 Training - All users of the program shall be trained in Ardent’s Behavior Based
Safety Management Program. Training will include the following components:
2.1.1 How to properly conduct a behavior-based safety observation
2.1.2 How to complete the observation form
2.1.3 What different behaviors indicate
2.1.4 How to conduct feedback sessions
2.1.5 Employees should be informed that they may be observed at any time
during their shift.

2.2 Feedback - Information developed from analysis of observations shall be shared


with employees to help improve safe work behaviors and eliminate at-risk
behaviors.

2.3 Progressive Discipline – Safety observations and employee interactions shall


be kept separate from Ardent’s Progressive Disciplinary Policy.

3.0 Responsibilities
3.1 Management – Ardent Management is responsible for the following:

• Ensure that the HSE Management System includes a Behavior Based


Safety Management (BBSM) Program and that the program is reviewed
and revised as necessary.
• Provide BBSM Training for all users of the program.
• Provide leadership and support for employees in communicating their
responsibility to stop the work when observing unsafe behaviors or unsafe
conditions.
• Provide resources to implement and maintain a BBSM program.

3.2 Supervision – Ardent Supervision are responsible for the following:

• Understand and enforce the BBSM Program and Policy.


• Participate in performing behavior-based safety observations by
completing 2 observations per week.
• Evaluate and provide feedback to other participants.
• Develop corrective action plan to prevent recurrence of unsafe behaviors.
• Inform employees that they may be observed at any time during a shift.

3.3 Employees – Ardent Employees are responsible for the following:

• Stop unsafe behaviors and act to prevent recurrence.


• Participate in observations by providing meaningful feedback to observers
when questioned about work procedures, behaviors, and job site
conditions.

4.0 Reporting and Recordkeeping


4.1 Custodian – Director of HSE or designee shall be custodian of all BBSM
records.

4.2 Control – Records associated with this program shall be handled in the following
manner.
4.2.1 Observation Cards – Cards will be collected weekly by site
supervisors. Cards will be forwarded to project or business unit offices
and the Corporate Safety Department.

4.3 Retention - Records shall be retained as follows:


4.3.1 Observation Cards – Will be retained until data is entered into the
share point BBSM database.

5.0 Corrective Action


5.1 Trend Analysis-Data obtained from observations will be trended
monthly to provide a basis for the Corrective Action Plan.

5.2 Corrective Action Plan- A plan will be implemented to reduce


reoccurrences of unsafe acts and revise safety training efforts.
