TO: ALL PMIers UNDERGOING SHIPBOARD TRAINING & THOSE WHO COMPLETED THEIR

ACADEMIC REQUIREMENTS(CAR)
Please take note of the following documents(forms) below.
1. CHED-MARINA CERTIFICATE OF SEA SERVICE(SINGLE VESSEL BOARDED)
2. CHED-MARINA CERTIFICATE OF SEA SERVICE(MULTIPLE VESSELS BOARDED)
3. IMO CREWLIST(for international vessel)
These documents should also be complied with in addition to your company's " Certificate of Sea
Service".
Additionally for domestic vessels, besides the "domestic crew list ", please comply also the (MDSD)
Master's Declaration of Safe Departure. MDSD should be certified true copy. In totality, there should
be 12 MDSD and 12 crewlist - one each every month.
In case your vessel is at "dry dock" or at " anchorage" for a duration of more than one month, thus
there is no MDSD-CREWLIST for that particular month, be sure to have a "certification" signed by
the master or the shipping company's president/manager( or its duly authorized representative) that
your vessel is at dry dock or anchorage as the case maybe.
The above dox shall be validated once you file for your BS,
4. It is mandatory to ENROLL your SBT-shipboard training(partial BS) before your embarkation(or
prior SBT), the enrollment of SBT will be reported to CHED/MARINA. If you are onboard undergoing
shipboard training be it a 12- month or 36-month category, you must enroll NOW your SBT. If you
have no time to enroll personally let your relatives do the enrollment here at STO Office. Just send
your documents(proof that you are onboard like: embarkation order, contract or agreement(for
international voyage only), SIRB entries, certificate of sea service, crewlist, etc.), payment for SBT
enrollment: Php 500.00 to be deducted against your BS fee once you file the same.
Failure to do so, is a denial for your BS.
Further, pls comply the TRB - including the "Sea Projects(your drawings and descriptions must be
signed by the master or the chief engineer as the case maybe, refer to Section PROJECT WORK its
introduction and instruction)" and Daily Journal( also posted for your reference: both deck and
engine)
( a sample of MDSD, with the attached MASTER's Oath and Crewlist for your guidance is posted
below)
DOX REQUIREMENTS FOR BS:
1. For Domestic Voyage:
1.1. SIRB 1st page and all entries
1.2. TRB (properly filled up, stamped and signed by the officers)
1.3. Project Works(drawings) signed by the master or the chief engineer
1.4. Daily journal (not less than 180 days) with crewlist attached
1.5. Certificate of Sea Service(Company) - notarized by a lawyer
or the new CHED-MARINA FORM
1.6. MDSD-CREWLIST(12 pcs.- one every month) certified true copy signed
by the master and coast guard and notarized by a lawyer.
1.7. Master reply slip (if you failed to send it during the duration of your SBT)
1.8. Quarterly evaluation form (if you failed to send it during the duration of
your SBT)
1.9. Picture 2X2, in white uniform blue background
1.10. STCW trainings and their COPs
1.11. TOR,CAV,NSO
2. For International Voyage:
2.1. SIRB 1st page and all entries
2.2. TRB (properly filled up, stamped and signed by the officers)
2.3. Project Works(drawings) signed by the master or the chief engineer
2.4. Daily journal (not less than 180 days) with crewlist attached
2.5. Passport 1st page and all entries
2.6. Certificate of Sea Service(Company) - notarized by a lawyer
or the new CHED-MARINA FORM
2.7. IMO crewlist - notarized by a lawyer
2.8. Master reply slip (if you failed to send it during the duration of your
SBT)
2.9. Quarterly evaluation form (if you failed to send it during the duration of
your SBT)
2.10. Picture 2X2, in white uniform blue background
2.11. STCW trainings and their COPs
2.12. TOR,CAV,NSO

2.1. SHIPBOARD TRAINING AGREEMENT FOR CADETS ENGAGED IN
INTERNATIONAL VOYAGE or CONTRACT OF EMPLOYMENT(POEA's
FORMS) or
2.2. travel itinerary and other similar documents
2.3. TOR
2.4. CAV
2.5. SIRB( first page)
2.6. PASSPORT( first page)
2.7. TRAINING CERTIFICATES

A "SHIPBOARD TRAINING PLAN" for Deck and Engine is posted in this page for your guide and
reference (please download it) regarding your structured shipboard training.
CMO 70 s. of 2017,stipulates that you must complete your shipboard training either 12 months or 36
months category and file for BS within six (6) after CAR. Otherwise, if you file beyond the said period
you shall refresh and update courses set forth by STCW and relevant national policies, standards
and guidelines.
To avoid inconvenience with the above mandate, make sure you can file your BS within the said
period.
