 Case Study
Mr. Henderson a 60 years old a man admitted to the hospital because of dyspnea. He
complaints of breathing difficulties and unable to remove th secret, he looks weakness,
pale and cyanosis oh her lips. From the physical findings : BP : 90/60 mmHg, RR :
30x/minute, P : 100x/minute, T : 37°C. He said he had a history of Chronic Obstructive
Pulmonary Dissease for 5 years, from the auscultation you heard ronchi on the basal area.

Base on data presented on Mr.Henderson

 Develop strategic communication plan for :
1. Teaching Affective Cough
2. Delivering Oxygen

STRATEGY IMPLEMENTATION

A. Nursing Process
 Client Conditions
a) Subjective Data
- Client complaints of breathing difficulties and unable to remove the
secrets.
- He said he had a history of chronic obstructive pulmonary disease for 5
years.

b) Objective Data
- He looks weakness, pale, and cyanosis oh her lips.
- Physical findings : BP : 90/60 mmHg, RR : 30x/minute, P : 100x/minute,
T : 37°C.
- From the auscultation you heard ronchi on the basal area.
c) Diagnosing : Ineffective Airway Clearance and Ineffective
Breathing Putter
d) The Purpose of nursing : To Remove a Secret, and To need oxygen supply.
e) Intervention : Helps Cough Affectively, and Delivering Oxygen

B. Orientation Phase
“Hello, good morning Mr?” “My name is Nurse Bella, I’m nurse on duty today from 7
a.m to 2 p.m”.
“What is your name?” “How are you feeling today Mr. Henderson?”
“Oh, you had can’t cough and unable to remove secret, Okay now I’m going to helps
you cough affectively”. “So the purpose to take out a secret”. “Its take about 15 minutes,
Its just here, do you agree?”

C. Work Phase
“Okay I want to prepare the instrumen”
 “I’m going adjust your position to sit on the edge of the bed of chair with legs supported.
I’m going tp place this towel around your neck and you can hold tissue clean your
secret”
“Do you want to drink warm water?” “okay, place your palms down on the ribs line”.
“Encourage the client to take a deep breath slowly, holding for about three seconds and
slowly removing it by mouth, Do it three times and the lost you can cough not exhale”.

D. Termination Phase

A. Orientation Phase

“Okay Mr.Henderson, I need to help you to Delivering Oxygen with Simple Mask, you
okay for that?”
“The time is about 5-10 minute for the place is in this room”.
“The purpose is to need your oxygen supply”.

B. Working Phase
“Okay, Mr.Henderson, First I will to change your position with s what emi fowler
position, so you can breathe well”.
“After that I will connect the oxygen hose and simple mask to the humidiflier, and I will
adjust the according to what you need”.
“Then, I will place this simple mask on your nose and your mouth”. “then rubber to your
head”.
“You can stay this simple mask and breath well”

C. Termination Phase
“How are you feeling after I delivering oxygen?”
“Do you feel better?”
“Okay, Mr.Henderson, It’s finished, If you need to help, you can press this button at the
bed side”.
“I will come back again to give you medicine at 12:00 pm, Okay see you later”.
