Plaintiff, through the undersigned counsel, unto this

Honorable Court, respectfully manifest that;

1. On April 12, 2019, plaintiff received the Order 1 of the

Honorable Court dated March 27, 2019, denying the

admission of Exhibits “A” and “B” including its submarkings;

 2. That the original office copy of the said exhibit “B”

which is the Delivery Receipt No. 27565 was lost and its

whereabouts can no longer be retrieved nor found;

3. That plaintiff’s Credit and Collection Analyst, Darius

Letada, the sole and official custodian of the same document

certifies the fact of such loss. Attached herewith is the copy of

Sworn Affidavit (Certification) as Exhibit “B” and made a part

hereof;

4. In view of such, plaintiff is hereby submitting a

Certified True Machine Copy of the same Delivery Receipt to

support our offer of Secondary Evidence;

5. This Manifestation with Motion to Admit Evidence is

being submitted in good faith and for the administration of

justice to be duly and properly served.

PRAYER

WHEREFORE, premises considered, it is most

respectfully prayed of this Honorable Court that the instant

Manifestation with Motion to Admit Evidence be GRANTED,

and Exhibit B be ADMITTED as evidence.
