mentioned below) on 22nd October 2019 (Tuesday) from a recognized hospital in your vicinity on an
empty stomach.

Cost of the following tests you will be reimbursed, upto Rs.2000, based on actuals per original bill
submitted at the time of your joining.

You are requested to conduct the below mentioned tests:

1. Hemoglobin

2. Blood Sugar (Fasting & PP)

3. ESR

4. Routine urine Examination

5. General information (As per format attached)

6. X-Ray Chest

7. Blood Group (Essential)

Attached L&T Medical Form is required to be filled in by the Doctor.

You are requested mail us scanned copy of reports and medical form at the earliest, to proceed with
recruitment process.
