CLINICAL PERFORMANCE EVALUATION CHECKLIST


(Immediate Care of the New born)

Instructions: Check the appropriate column. Check column “YES” if the behaviour/competency has
been observed . Check the column “NO” of identifies behaviour/skill is not demonstrated. Specific
remarks may be written in the column provided, especially for answers that are neither in the YES or NO
columns.
