 Go to Pasay Cityhall.
Goal – To get 2019 fire permit without paying penalty or additional cost for GRAND88 TAIWAN
VENTURE INC

Procedures:
1. Ask the BPLO why fire permit was not provided during January 2019 business permits
renewal.
2. What is the process to get a new one.
3. Go to Sanitary department, look for George, show the sanitary permit of Grand88.

 Call Mark Isidro – Owner of Ride Safe, ask him if he has the certificate already.
 Contact 09178050070
 If the Certificate is already with him, please meet him at greenhills and proceed with the
registration. If NO certificate yet, proceed with Paranaque City hall and process Cuatro
documents.
 Get Business permits for Ride Safe Enterprise in San Juan City
Documents Needed:
1. SEC Certificate of Incorporation, Please photocopy in the office the SEC.
2. Certificate of Condominium Title, please photocopy in the office
3. Sketch of Condominium
4. ID of Mark Isidro
5. Authorization letter

Procedure:
1. Go to the office first, download the following documents above in our company email
address

2. After you process KZONE BIR document, go to Greenhills Brgy Hall and get for Brgy
Certificate
Address: Greenhills Barangay Hall, 1204 Wilshire Plaza San Juan .

Procedure: Pay for the Brgy Clearance. Will transfer to you the amount.

3. Go to Cityhall of San Juan and register for Business Permits.

 Procedures:
Secure Locational Clearance
 Fill up the Locational clearance form
 Paid up capital is only 400K
 Not renting, office is condominium. Please show the condominium certificate

Get Public Liability Insurance

Community Tax Certificate (Cedula)
Fill up business permit application forms
Get Fire Permit
Sanitary Permit
Pay for the assessment.

 GOAL: Update the Business Address of Cuatro probuilders Inc.

Go to Paranaque Cityhall, as the BPLO how to change business address
Get the requirements. If you can start the process today, please do so. If you do not have the
complete documents, proceed to BIR Sucat RDO 52.

Go to Paranaque RDO 52 -Sucat.

Get BIR for 1905
Fill up BIR form 1905.
Fill up the following in 1905
1. Part II Change in Business Address
2. 4E Change in business Address: Transfer within same RDO.
3. New Address: 21 Saudi Arabia BETTER LIVING SUBDIVISION DON BOSCO PARANAQUE
CITY
4. Sign the form
5. Submit to RDO.
6. Ask the RDO officer on the next steps for changing business address.
7. Pay the applicable fees.
