AFFIDAVIT OF LOSS

I, LOIDA GONITO BANAAG, of legal age,

Filipino, and presently residing at Brgy. Pallocan
West, Batangas City, after having been duly sworn to
in accordance with law hereby depose and state:

That I am a customer of MERALCO and as such,

I was issued the corresponding Contract for Electric
Service with its branch in Batangas City under
Service ID number 770247401-1;

That sometime on March, 2013, while I was

going over my documents, the envelope which
contained said Service Contract was already
missing;

That diligent efforts were already made to find

and locate said envelope and service contract but
the same were all in vain;

That this statement is executed to attest to the

truth of all the foregoing facts and circumstances, to
inform MERALCO of the loss of my Service Contract
and for whatever legal purpose this may serve.

IN VIEW THEREOF, I have hereunto set my
