Madam:

Greetings of peace and joy.

I regret to inform you Madam that I cannot attend the Trainings on

Partnership Management and Negotiation Skills for School Heads in Cebu on
September 24-25, 2019 due to medical reason. It was last August 2019 when I
experienced difficulty in breathing and falling asleep. I consulted Dra. Ezra
Sevilla of Mary Johnston Hospital and advised me to have ECG of which was
found negative of heart problem so I was given a request to have full abdomen
CT scan and was diagnosed of fatty liver due to high cholesterol so I was put in
medication for my acid reflex and melatonin for sleep disorder for a month and
need another laboratory test on December.

I am apprehensive Ma’am because of the distance of the venue of the

seminar and worried for my sleep disorder too.

Attached herewith are the result of my CT scan and prescription of which I

started to take this September, the melatonin because of the acid.

It is hoped that this request of mine be given your utmost understanding

and consideration.
