0021499161 and telephone number 0495361491. I wrote this letter to request from your
good office for rebate for the amount we paid from June 2019 to August 2019 due to
non-service of phone line from June 21, 2019 up to this moment, October 2019.

Relative thereto, I would like also to request to authorize Jocelyn C. Ada, our housekeeper
to terminate or disconnect my account in your company effective today due to non-
service of our PLDT Landline. Our housekeeper reported it to your company but there was
no actions.

I hereby allow Jocelyn C. Ada to sign any documents regarding this request on behalf. I
have no objections on this authorization.
