1. Provide nursing care that includes Satisfactory Progress (SP) [3 pts]


comprehensive, collaborative Evidence/Indicators: I was assigned a patient who
assessment, analysis, planning, was admitted the previous night for a stroke.
evidence-informed interventions and Throughout my shift together the nurse and I
outcome measures. completed ongoing assessments and assisted in
prepping the patient for tests. Our collaborative
assessments included the patient, her husband
and children, and other specialized members
within the hospital. Ever 30 minutes we
completed the Canadian Neurological Stroke
Scale. With our findings we analyzed the data to
determine that effects that the stroke had to the
patient including memory, motor movement,
speech, etc. With these findings we planned our
day and how we could assist the patient in
completing some ADLs such as bathing. With
assisted the specialist is completing a barium
swallow test, from these findings we determined
that the patient could only be on clear fluids,
knowing this I used the evidence-informed
interventions on how to provide the patient with
nutritional needs.

1a) Interpret critical aspects of the Satisfactory Progress (SP) [3 pts]


person’s experience of acute illness in Evidence/Indicators: I was assigned a post-
relation to common signs and symptoms, operative patient who had their small bowel
responses to treatment, patterns of resuction. The patient was alert in the three
coping, and impacts on individual and spheres and was able to comprehend
family relationships conversations. This interaction was benefical to
myself as a nursing student as I was able to get a
full sense of acute illness in post-operative
patients, as the patient was able to interact with
me and tell me everything she was feeling. The
family was very involved in the patients care, this
was also benefical to myself as I experienced how
to interact with not only patients, but also there
family members. Most of the family members did
not understand the medical terms that the doctor
was using, this was an opportunity for me to
assist the family and the patient in understanding
everything that they were informed of. The patient
was in a severe amount of pain and was
prescribed a PCA pump, this was a good
opportunity for me to educate the patient on how
to use the pump in an effective and safe way. In
educating the patient about the pump I was able
to help her manage the symptom of pain that she
was experiencing.
1b) Demonstrate selected nursing and Satisfactory Progress (SP) [3 pts]
collaborative interventions related to Evidence/Indicators: Throughout my placement I
caring for adult patients with acute illness. have had a large scope of patients who have
Pre and Post-op care, IV medication, acute illness. Therefore, I have been able to
physical assessments, wound care, practice and demonstrate many nursing skills
rapidly changing conditions, and within placement. I have been assigned a post-op
resuscitation. patient who I provided post-operative care for.
When caring for this patient I got the experience
of reconstituting IV medication as well as
administering them, I performed an abdominal
dressing change, as well as did ongoing
assessments to assess the patients post-
operative state and pain. Each week at clinical I
complete a head to toe assessment on my patient
and document and report my findings to the
primary nurse. The unit that I am on is a rapidly
changing environment as many patients are
being admitted or discharged, working along side
a nurse whom is caring for four patients, has
helped me to develop skills in working in a
changing environment, as we are constantly
caring for one or more patient who all have
different types of acute illness.

1c) Identify common medical treatments Satisfactory Progress (SP) [3 pts]


and potential Evidence/Indicators: Each week that I am at clinical
consequences/complications of selected I learned the medications that my patient is
acute illnesses taking, as well as any adverse reactions that can
occur from the medications. It is important to
understand the adverse reactions that can
happen for medications in order to detect if they
are occurring in our patient, so we can intervene
and make changes when they occur. Last week at
clinical I inserted a NG suction into my patient
with the guidance of my clinical instructor. Before
I inserted the NG I learned how to insert the tube
while following the best practice guidelines as
well as complications that can arise from the
insertion of an NG suction. In doing this research
I learned the importance of making sure than an
NG suction is in the patients gastric cavity and
not their respiratory airway as this can develop
complications in the patient.

1d) Demonstrate health promotion and Needs Development (ND) [2 pts]


illness prevention practices. Evidence/Indicators: Before I walk onto the unit
each week I ensure that I sanitize my hands.
Before I go into a patients room or leave the room
I ensure that I sanitize my hands. When disposing
of bodily fluids such as gastric content, blood,
etc. I use the correct biohazard bin to dispose of
the content. When moving from room to room I
ensure that I wipe down any equipment that I
have used such as my stethoscope or blood
pressure machine. One thing that I can work on is
when I am in patients rooms try to ensure that I
do not touch things such as the draw curtains or
bed when I wearing gloves that I have
contaminated. With regards to health promotion I
try to encourage patients who are able to use the
hand sanitizer located on their bed table before
meals, after the washroom, or before and after the
leave the unit.
1e) Engage with patients and families to Satisfactory Progress (SP) [3 pts]
identify health-related situational Evidence/Indicators: I have engaged with many
challenges immediate family members so far while at clinical.
During my first week I was assigned a patient
who had a stroke the previous day. Her and her
husband had been married for 50+ years, so this
was not only a situational change for the patient,
but it was also a large change for her husband.
Due to the stroke the patient was unable to talk,
therefore to learn about the patient I interacted
with the husband to gain some health history on
the patient as well as any interests or dislikes she
has. This was not only beneficial to the nursing
staff, as they learned about the patient, but it also
helped the husband talk through his feelings and
emotions in a supportive environment.

1f) Work with patients and families to Satisfactory Progress (SP) [3 pts]
create reasonable and effective solutions. Evidence/Indicators: While at clinical I have had
some patients who have not wanted to take part
in treatment or care, specifically when it comes to
personal hygiene. I was assigned a patient who
had not showered in 4 days as she did not have
her own soap and did not want to use the
hospitals. Knowing this, the following day when
the husband came by to visit I asked if next time
he could bring in some of her personal hygiene
products. Later that day the husband returned
with shampoo and body wash and I was able to
assist the patient in having a bed bath.

2. Establish and maintains therapeutic, Satisfactory Progress (SP) [3 pts]


caring and culturally safe relationships Evidence/Indicators: I have cared for a patient who
through effective communication. has not been able to communicate through
speech due to her acute illness. When interacting
with this patient I ensured that she knew every
step I was doing when caring for her or
performing assessments on her. I knew that the
patient understood what I was doing as I asked
her to shake her head yes or know if it was okay if
I performed care or if she understood or not.
Even though the patient could not verbally speak
I was able to effectively communicate with her
through non-verbal actions. In doing so this
provided the patient with therapeutic care in a
safe environment.
2.a) Explain the experience of acute Satisfactory Progress (SP) [3 pts]
illness in individuals receiving care in Evidence/Indicators: Many individuals whom are on
acute setting my unit do not have a long duration of stay,
therefore it is important to educate the patients as
much as possible about how to look after their
health and maintain their health when they return
home. Many individuals who experience acute
pain experience pain and a change in health
status, therefore it is imperative that we not only
provide them with physical interventions to treat
their illness but also provide them with emotional
and psychological resources. Many individuals in
the acute setting have a recent onset on their
illness, and do not know much about their illness
or the treatment they are receiving, it is our jobs
as nurses to ensure that the patients are aware of
the treatment they are receiving such as
medications or tests.

2.b) Demonstrate therapeutic use of self Satisfactory Progress (SP) [3 pts]


Evidence/Indicators: I have demonstrated
therapeutic use of self as clinical by being aware
of myself and my feelings and or emotions. I try
my best to not let my personal life affect my care
and work while at placement. Therapeutic use of
self is important to make sure their is no power
dynamic between myself and the patient, to do
this I ensure that I am talking with the patient and
not over them, I use terms that they will
understand, and I ensure the patient that I am
there to support any of their needs. In
understanding therapeutic self I am able to
communicate with my patient and gain their trust
my learning their likes/dislikes and or triggers.

2.c) Understand and anticipate emerging Satisfactory Progress (SP) [3 pts]

acute illness and apply this knowledge to interactions amongst the patient and doctors or
care: Plan and predict outcomes of surgeons. Before these interactions happen I
nursing care, evaluate client response to encourage the patient to ask any questions to the
nursing care, demonstrate patient doctor as ask for something to be repeated in
advocacy. different terms if they do not understand it. I
encourage the patient to have a say in the care
they receive or not receive, for example any
interventions they do or do not want taken. In
doing so I advocate for my patient and ensure
that they understand the process and
interventions of the illness. When caring for
patients I learn their physical needs they might
have such as the use of a walker, etc.. I also learn
their psycho needs such as they may like me to
talk quietly or perform my skills slowly. I
understand the social needs of my patient by
understand the environment they like to be
situated in, for example some patients may not
like to have more than one nurse present in the
room at a time.
3. Apply the ways of knowing and Satisfactory Progress (SP) [3 pts]
informational technologies to effectively Evidence/Indicators: I have applied the ways of
care for diverse, acutely ill patients. knowing throughout my clinical so far, as I
engage in personal, empirical, ethical, and
aesthetic knowing. I have engaged in personal
knowing by engaging with my personal ethical
and moral considerations as well as
understanding what I know and what I can further
build and develop upon. I have engaged in
empirical knowing by understanding the science
behind nursing as become familiar with nursing
interventions and care. I have engaged in ethical
knowing through understanding what is right and
wrong based off of clinical standards and
practice and understanding my role in protecting
human life. I have engaged in aesthetic knowing
by understanding the art of nursing and
interacting with my patients in a meaningful way
that incorporates their personal feelings and art. I
have used informational technology by reviewing
sites that will further my learning while at clinical.

3a) Apply relevant nursing models, Satisfactory Progress (SP) [3 pts]


philosophical frameworks, theories and Evidence/Indicators: As previously discussed I
evidence. have applied the nursing model of Carpers Ways
of Knowing, but applying empirical, ethical,
aesthetic, and personal knowing within my
practice. I applied Nightingale's theory of
environment and nursing by creating a
supportive environment for patients, as
Nightingale found that personal health is
impacted by the environment in which we situate
ourselves in. I have applied Watson's theories of
health as I understand that health is not only
defined by the body, it also encompasses the
mind and soul.

3 b) Adhere to policies and practice of the Satisfactory Progress (SP) [3 pts]


institution. Evidence/Indicators: While at clinical before I
provide the patient with a nursing intervention
that I have not done before or that I am unfamiliar
with I review the policies and standards of the
hospital. For example, I have removed and
inserted a NG suction from a patient. As I had
never done this before, I went on to the hospitals
data base and searched for the policies on NG
suction. When searching for these policies I
learned to proper technique to follow based on
the best practice guidelines. Before I reconstitute
any medication, I ensure that I review what they
medication can be reconstituted with and if there
are interactions with other drugs. I review this
information on the computer at the hospital using
Lexicomp or in the IV drug binder located in the
med room.
3c) Use knowledge gained in theory and Satisfactory Progress (SP) [3 pts]
labs to support patient care decisions. Evidence/Indicators: I have used the knowledge
gained in theory courses to interact with my
patients in a way the encompasses their
psychological and social needs. For example this
year in NURS 3000 we have been learning about
the ways of knowing, throughout clinical I have
taken this prior learning and applied it to caring
for my patients. In lab I have learned clinical skills
and interventions such as dressing changes, IV
medication, assessment, etc.. Each week when I
learn something new in lab I try to transfer over
that learned information to my clinical practice.

3d) Uses best practice guidelines. Satisfactory Progress (SP) [3 pts]


Evidence/Indicators: To ensure that I was best
practice guidelines I review procedures or
processes that I have never done before or am
unfamiliar with before I perform the skills. I do
this by reviewing Lexicomp at the hospital or if I
am at home by reading the textbook. When using
Lexicomp I print out and review the step by step
process as well as watch informational videos on
how to perform the task. All of these resources
meet the best practice guidelines. If I do not know
the best practice guidelines I will not complete a
task or ask the nurse to complete it for me until I
understand the best practice guidelines.

standards and organizational polices to Evidence/Indicators: I adhere to professional
contribute to a culture of safety. practice standards and organizational policies to
promote safety for my fellow nursing staff and the
patients whom I care for. Some examples or
organizational policies that I follow while at
placement are adhere to the patients transfer
status, for example if the patient is a mechanical
lift I will not transfer the patient without a nurse
whom works at the hospital with me. Another
example of how I follow practice standards is by
completing my clinical skills such as medication
administration with my clinical instructor, as this
not only protects the patient but it also protects
myself as a nursing student.

4a) Critically appraise own practice in Satisfactory Progress (SP) [3 pts]


relation to nurse-client/family interactions Evidence/Indicators: I critically appraise my own
and as a member of the health care team practice in relation to nurse/client/family
interactions by ensuring that I am following the
best practice guidelines when performing care. I
ensure that my patient is informed of what is
happening for the day, however I will not convey
information to the patient that I outside of my
scope of practice. For example, I will not tell the
patient diagnostic results as that is within a
doctors scope of practice. When interacting the
the patient, family, or other members of the team I
am conscious about what I saw to ensure that it
is respectful and appropriate for within the
workplace.
4b) Engage with patients in an ethical and Satisfactory Progress (SP) [3 pts]
culturally safe manner Evidence/Indicators: I have had patients whom
express their own ethical, political, or religious
views. Even though some of these views may
differ from my own I also am supportive of the
patients views and engage in conversation with
them in a positive manner. At clinical I support all
patients with the same dignity, respect, and
quality of care no matter their ethnicity, religion,
gender, etc. as all patients should be treated
equally.

4c) Demonstrates accountability Satisfactory Progress (SP) [3 pts]


Evidence/Indicators: I demonstrate accountability
each week at clinical by showing up on time and
in proper Trent Nursing uniform. I have all my
prep completed for each week and review my
patients diagnoses and or medication. I ensure
that my patient is safe and had care performed on
them before I leave the unit each week. I also
demonstrate accountability when interacting with
my assigned nurse as I let her know my findings
regarding my patient and ensure that she knowns
I can be of assistance to her when caring for
other patients.

4d) Demonstrates reliability Satisfactory Progress (SP) [3 pts]


Evidence/Indicators: I demonstrate reliability while
at clinical by fulfilling my obligations as a student
nurse. My obligations include documenting my
findings, ensuring that my patient is safe and
cared for, as well as reporting to my assigned
nurse and clinical instructor. I am reliable as I pay
attention to detail, I listen to morning report and I
pay attention to my patients status and any
interventions they may have such as IV
medication. I also demonstrate reliability by
following guidelines and policies as our patients
rely on us to provide them with the best possible
care while following the set standards.

5. Exercise leadership to enhance patient Needs Development (ND) [2 pts]


care, and support professionalism in Evidence/Indicators: I try my best to demonstrate
practice. leadership while at clinical, however this is
something that I find hard to do as I sometimes
get nervous as a leader, however I have come a
far way since first year and I hope to continue to
develop upon these skills. I demonstrate
leadership by assisting my fellow nursing
students and assigned nurse with daily tasks or
when they need assistance. I am a leader as a
clinical student as I help my peers review and
understand policies or guidelines that they are
unfamiliar with, as this enhances patient care.

development based on reflective practice Evidence/Indicators: I participate in professional
and critical inquiry. development as a nursing student by learning
from my patients, clinical instructor, assigned
nurse, and fellow peers. I reflect on my practice
each day of clinical when I get home to determine
my strengths for the day and concepts or skills
that I should continue to build upon. I engage in
critical inquiry by taking personal feedback as
well as feedback that I receive from my clinical
instructor. I do not take any feedback in a
negative way, as feedback is what helps us to
learn and grow within the nursing profession.

Evidence/Indicators: Each week at clinical I
support my peers in a professional way. I offer my
help when needed and always feel comfortable
asking my peers for help. I support my peers by
having a listening ear if they need to take about
their own personal feelings or about the nursing
job. I support my peers by providing my guidance
of knowledge about a topic when they are unsure
or need guidance. I support my peers by
assisting them in completing daily tasks. For
example a male peer asked if I could assist him
with a bedbath. I was doing one at the time, so I
told my peer that I would be 15 minutes longer
and I will come help once I am done. My peer
needed assistance as the bedbath was on a
female patient, so I offered my assistance.
Supporting peers is an important aspect of
nursing as nurses work together, we do not work
independently.
