In the event of a medical emergency, please refer to the medical emergency card and call Chubb Business

You will need to provide Chubb with the following information:
Your date of birth
Your home address, mobile telephone number if available and email address
Oil Consultants’ policy number: UKBBBC71913
The name of the hospital where you are receiving treatment
The name of your doctor and contact number if available
If you are unable to phone, please pass this information on to the hospital so that they can phone on your
behalf.
You will be given a claim number – please pass this number to Oil Consultants as soon as you are able to.
If you are unable to inform the client that you are unwell, please let us know so that we can inform the client on your
behalf.
